The final design of the badge has now been determined. You can see it by clicking on the spoiler button below:
Distribution will be via unofficial social events such as the WCTT, CST and the Pre PAX Dinner or from the BYOC registration desk at PAX itself.
The names on the back of the badge have not as yet been collated but will be towards the end of July. To get your name on the badge you have to participate in this thread, be an admin of the forums or take part in one of the unofficial social events that has badge distribution associated with it.

I'll need to get clearance on the use of the PAX logo. It got me into trouble back in 2008 with Mr Khoo, so I'll have to clear it with him and/or his subordinates before we can use it. Otherwise we'll go with 4. Thanks for the feedback everyone.
Also, where are you going to be passing these out this year now that the Pre-PAX dinner is no more (sadfaic)?
I really want to like 3, but the lens flare is just distracting to me.
Also, Khoo usually is cool about things like this, but don't think that means he will allow the use of the PAX logo. I only say that because he told my team we couldn't use it. That's why I'm glad you're going to ask before making anything final.
So I'll actually detail my opinion a bit more: 1 is good and seems to be the favorite. I'm also fine with 3, minus the lens flare. I won't choose 4 or 5 because I personally don't like the swirling background. And the only thing I would want different from the others is the removal of the red parts of the background. I just think it stands out a bit much and should be all blue to retain the Prime feel.
But that's my opinion. I will be happy with whatever I end up with when they're done.

The ones with Lens Flare are bad.
If you cannot use the logo, I'd go with #3. The lens flare does not work as well with the PA logo in my opinion.
Also, we going to have names on the back again cause that was cool.

I also like the Penny Arcade one Sans lens flare, because since it says PAX forums, I feel that PAX Pax forums is redundant.
