I never once got sore balls. I did fall on my head a couple of times and got some blue toes after being stepped on and I guess maybe your like inside thigh muscles might hurt a bit. Maybe your like ass bones (I'm assuming that is what they are called) might be a bit bruised as well.
But for the most you don't get injured. Just try not to piss the horse off.
