I think Dr. Nooooo is something that has been done to me a couple of times. I've died a bunch of times with full übercharge, some of them have got to be from backstabs.
It's one of the achievements. Something to do with pyros, obviously. Probably killing one after being set on fire by it.

On a related note, me and a buddy set our backstab records today by running a stab train on Dustbowl. We'd start at the blue exit behind the shed stab everything, run into the cap point, recloak and run back to the shed. Somehow they never managed to catch on where we were coming from. I got a record of 9 and my buddy a record of 8. It was like a goddamn assembly (or disassembly, oh ho ho) line of carnage.
Are we all saying that we'll have unlock milestones still or is there going to be something different this time around?
I mean, I get the bitching about having to grind achievements to unlock weapons, etc, but with two classes coming out at the same time, the reasons why that's uniquely bad are mostly lost.
"The Melbourne Supremacy" is probably just to dominate a sniper.
My guess on "May I Cut In?" is to kill a medic and his heal target within 30 seconds, that or kill his heal target then get healed by the medic.
"Insurance Fraud" probably requires getting healed by an enemy medic to some extent.
If you can't equip it through the menu, you can't equip it at all. Sometimes servers just lose connection to Steam and prevents the use of unlockables.
