they did a bit of that pre-release too with dashing strike and crippling wave and such, and I believe they are adding fist weapons back in to all of his animations post-release as well

The other classes already had weapon animations. However, in the monk's case, it would be running around with a staff or whatever, then whenever you threw a punch or kick or something, the weapon would flash back to it's holstered state then reappear in hand after the attack.

The only time I'm 100% sure it's the character name is when there's an interaction with an npc, or your character does an emote it'll show their name before the text in the dialogue box.
i mean if you die because you fucked up or decided to charge into a boss pack or something then fair play take your lumps
but dying to circumstances beyond your control which can and will happen

not really that common of a thing. If you know you have connection issues you should fix them. And the lag works differently than d2. The like, 1 time I got lag in all my beta play time due to them stress testing one server after a large invite wave.
And permanent death is more fun in games where you can make drastic changes to your character. In D3 there isn't any "oh I died. Guess I'll take this opportunity to try out some new crazy build". You die with a demon hunter, you roll the same damn demon hunter.
