I go by "Incredible Chicken". My strategy is to go awesome adjective and mundane object/creature. During TF2 and L4D, most people just use "Chicken" for short. Some pyros who kill me think it's very clever to refer to me as fried chicken.
Chance was the name of my Elvaan Samurai in Final Fantasy XI - my first real foray into online gaming. When I moved up to WoW, Chance was already taken (went with 'Chances', sigh), but I ended up with a cool name for my sutblety Rogue alt: Splinter. Tell me that ain't the best name for a Gnome rogue?
Anyway, it makes my PSN name seem kinda' douchey. Chance and any variation thereof was obviously already taken, so I combined the two names and went with Chance_Splinter. Sounds like an accidental, incredibly minor injury.
But at least I miraculously got to keep 'Chance' for these boards. That means a lot to me, actually.
Edit: oh, and my favorite name that I've seen someone else use is a fellow Canuck on these boards - "Guidedbyvices". I fucking love that, and if I ever start writing again I will somehow make reference to someone who makes life decisions based solely on how it facilitates their addictions.

My name used to be Herby. When I was in high school (long ago), a few friends of mine would make posters with Print Shop on the original Mac - one I made featured Herby, the Unholy Woodchuck. I've kept the handle. When there was a name change thread, I was drinking and requested The Love Bug just on a whim - I got tails and I'm now Lindsey Lohan.
On the flip side - my handle on most other stuff is N8 - as my name is in fact Nate. My band name in RB2 is The Unholy Woodchucks.
I picked my name years ago when I joined the World of Monkey Island forum. My reasoning was that the Rhesus part kept the monkey aspect of the forum in mind, and the Positive came from the blood group. In the classic episode of Hancock's Half Hour, "The Blood Donor", Hancock gets briefly annoyed by the insinuation that his blood is Rh+, exclaiming, "They're monkeys, aren't they? How dare you!"
Since I have few good ideas, I never bothered trying to think of another name. I tend to shorten it to Rhesus elsewhere.
A long time ago on a MUD, I tried using a name. The admins denied it. So I tried the name Sypha. They accepted it. So henceforth I was known as Sypha. Because Sypha was a freaking awesome character that I remembered from Castlevania 3.
Apparently I didn't remember that Sypha was a female.
Then one day on the same MUD, a bunch of people started making -fire/-fyre names. Starfyre, nitefyre, blowmefyre, etc. (actually the latter didn't get accepted). Hence from then on I was known as Syphyre.
Which is cool, because it sounds like Psi-Fire. Which means I can set your brain on fire. I guess.
Same exact thought crossed my mind when I read that.

My name is a nickname my instructor gave me when I practiced Tae Kwon Do. I guess he thought I was a cocky kid or something. Pretty soon everyone at my school called me that.

Dr. Evil -> Evil -(Too many People Named Evil in games, needed a last name ala Dick Dastardly)-> Evil Badman -(Couldn't Recover Evil Badman from XBL)-> Evil Badman MD (Making me a doctor).
I had two 1 month trial alts named Evil Badman RN and EvilBadmanOBGYN.
The Raiden part comes from Virtual On, the robot game. WHICH PREDATES MGS2 BY YEARS THANK YOU VERY MUCH.
The 333 part comes because I first registered my name on various forums when I was young and stupid and liked numbers, and it's easier to have all of my names be the same now.
I wanted to avoid numbers in my names as much as possible. I used to post under Infil (my online name used everywhere else), but switched to XenoScholar for some reason. I don't even know where I came up with this.
Infil is short for Infiltrater [sic], a name I used to go under. I shortened it to eliminate the embarrassing spelling mistake, and because I like the sound of "Infil".
My online name is Infilament, because Infil was taken. I'm part of another community that started a meme by adding "ament" to everything (ie, "lol donkaments" if you know online poker). Plus, Infilament sounds cool, since it sounds like it also could have arisen by combining two unrelated words. Plus, I didn't want to add any numbers to the end of Infil for my XBL name.

Unfortunately, all that was available were some books by David Eddings.
When you've gone without sleep for a few days, even the stuff he writes seems like fantasy gold. Luckily most people don't get the name, so I don't get bothered much.
I usually go by Koro in places now, and that itself is pretty interesting in the fact that it's also the word some rural folk in Southeast Asia use for "genital retraction syndrome".

Back in the days before the internet existed as we know it now, I ran a local BBS. Being the Metallica/Megadeth fan I was in junior high and high school, I called it Tornado of Souls, and my sysop name was Polaris. The 314 I started using when running into cases where my name was taken, so nowadays I just add it anyway, since I'd rather have a consistent handle I can share between games/sites/forums/etc.
I started off with Kendo and Kendomagic, after my brother's friend used to call me that as a kid. (real name is basically Ken___, depending on who i'm talking to). Then playing Diablo II i came up with Billington, because i thought (think) it is hilarious and I still use it regularly. Back in the old UO days, my brother played as Kid Rock and I was Mel Gibson. On occasion we'd also have a thief named ODB. I stuck with ODB and Mel Gibson through most of my online playing days because they are awesome, until I was playing free UO shards just a couple years ago and needed a new name for my PK, and thought Flanky sounded neat.
Cue Steam, and TF2, ended up as Dr. Flanky. I kinda switch between the two now.

elevature is the name of a song and EP by a band called the Flashing Lights. I loved them back in '98 or so, which is when I started using this name online. I'm not a fan any more, but I've registered this name everywhere and it would be a huge pain to change it (and in some places, like here, it's not changeable at all). Anyone with a pot reference or 420 in their name gets muted. Pot is great, but the people who use it as online names are universally douchebags.
One of the best names I've seen online is omgboomlol, who posts on the platformers forum and may post here. It's really the combination of the name and the bob-omb av/sig he uses that make it great.
My other favourite is SpunjBaab. I played with him once in UT99 ages ago, and I never forgot that name. If I could easily change all of my online names, I'd steal that one.
Oh also, a name i've liked, I played UO with a guy named WrathOfGod. A super lame name. But then I added him on AIM, and his AIM name was AMindAtLarge.

I do the random keyboard thing to name my characters in Nethack - most of them don't survive to Level 10, so there's not much point in caring what they are called. The thing is, my typing has reached the stage where I type groups of letters or whole words automatically, so the names always make some kind of phonetic sense.
For example: Squiggly, Milfinfar, Squagboar, Chillisoan.
Also, I tend to pre-judge people with ridiculous usernames, like the ones already mentioned. But if they are cool, using a mic, and arn't talking about being racist/talk (yelling) at their parents with the mic's still tuned on/being an asshat in general (all very likely on Xbox Live), then I will generally throw my assumption out of the window.

I really didnt have a name for games until I picked up an xbox and was thinking up a gamertag and wanted a jokey name, so I went with lookfreegrenade.
Short story: My name was chosen at random from a lost of Greek/Latin suffixes in the back of my 9th grade biology textbook, about 10 years ago.
Long story: I was creating a tabletop RPG world/short story/whatever thing with some friends at the time, and as character ideas, I picked a bunch of random words from said textbook. Names like Glykys, Caedere, Spirare, etc. all became characters I ended up using. Because I was a geek (and a furry to boot), it ended up being an anthro RPG thinger.
I moved away not long after, but I was attached to Caedere in particular as a character, so I worked on developing more and more backstory over the years (right now I have outlines for a few novels and a shit-ton of notes). To amuse myself, whenever I'd play an MMO or RPG or whatever, I'd recreate Caedere for that particular game. I have Caederes everywhere now.
Now, "Caedere" isn't my primary screenname, but my main one screams "HOLY SHIT GUYS I'M A FURRY" way too much, so I just register as "Caedere" for any non-fur fandom websites.
I'm TrueCaedere on Xbox Live, because I registered "Caedere" on the original Xbox and forgot all of the login info. "TrueCaedere" is a pun - it sounds similar to "Trucidare", which has a similar meaning to "Caedere"; "Caedere" means "to cut" or "to kill", and "trucidare" means "to massacre".
What pisses me off is that I was the only Caedere anywhere for a good long time, until some godawful death metal band came around and chose that as their name....
