MMOs usually give you an unlimited number of chances to get an item, though. It might not be the one you are looking for and so you're partially bound by luck, but you can always retry the dungeon/boss/whatever for another chance.
It's the same basic premise though, force items to be rare by only creating a limited number of them, or force items to be rare by only awarding one 0.001% of the time. Not a big deal in games these days, but in the Everquest era, people would pay truly amazing amounts of money for items that no longer dropped.
It's kind of entertaining to watch the TF2 team go through the same learning experience MMO developers did nearly 10 years ago.
Man it's been a long time since the last class patch. I hope all these little baby patches are their way of buying time for the ultimate Triple Class Update. That would be the best way to fix the bullshit of an update involving the Engie - just one class to counter them isn't enough.

The updates will pick up once L4D2 is live.. which is very soon now. We'll probably have the next class patch early next year.
The mini updates are merely little quickies they can toss out to keep people interested.

- In the nondescript past, Zephiniah Mann, taking the advice of his sons Blutarch and Redmond, emigrates to America to buy huge acres of "valuable" gravel. He takes his Maidservant Elizabeth, his tracker Barnabas Hale and a mysterious third person.
- The gravel turns out to be a waste of time. Angry at this, and for being inflicted with every disease one can get in Victorian times, he punishes his sons by leaving his worthless purchases between them.
- Elizabeth gets his money and tobacco plantation.
- Barnabas Hale gets Mann Co.
- The mystery man gets a mysterious thing.
- Redmond founds a company.
- Blutarch founds a company.
- Redmond and Blutarch's companies do battle with each other, continuing the senseless bickering their founders had in life.
- Elizabeth's descendant(s?) makes use of the tobacco plant and presides over the fights.

- The Mysterious Man does something mysterious with his Mysterious stuff.

The 2 sons he mentions are in the picture, both representing a sibling rivalry (red vs. blu). I wouldn't be surprised if they both grew up to both run the companies RED and BLU.
