One of the physical skills is the standard route, they buffed immolation arrow a bit this patch though.

If you don't want to go the standard route, I'm a big fan of of the hybrid build (that you've already done half of) maxing lightning fury and charged strike. LF is your crowd control, CS is your boss killer. The gear is cheap too, you can use any old javelins, and your resists should be fine since you're not using a shield anyways.
Ok, imma start a game on East, I like to take things at a steady pace.

Yes, that works all the way to Perfect (3 Flawless = 1 Perfect). Only the mid to high runes have additional ingredients.

Next: If I transmute two Amulets, one with +3 Energy and one with +17% Poison Resist, will I wind up with an Amulet with both? One with something completely different? Can it even be done?

Item transmutation recipes always gives you a new reroll with random stats. The only thing that matters is the item level(s) of the original items. You'll have to check the arreat summit or the diablo wiki for the exact formulas.
If you want an amulet with certain stats, you'll have to either trade for one, gamble a lot of gold on amulets, or item find for a long time until you get what you want.

I used the respec thinger to change over to Holy Shock. It got the job done, but I was still brutalized by Duriel and was putting little tiny dents into him. Barely an improvement over Holy Freeze's performance.
Now I'm in act 3 with the goddamn fetishes running around willy-nilly, not being slowed down.
Duriel on normal difficulty is usually the most difficult, since you don't have any efficient damage skills and your equipment is generally poor. NM is usually easier since you have better equipment and a few maxed skills. Depending on your equipment, hell may be even easier, or it could be ridiculously difficult

Is there an option to have item tags permanantely on, or do you have to hold alt to see them? It's about the only thing that bugs me about D2.
I'm guessing there's no way to do it as I couldn't see an option, but hey worth an ask.
I wish they'd port D2 to the DS along with 1,000 replacement screens with every purchase.
