There are two separate elected houses as part of a compromise reached between states with larger populations and smaller populations when the country was founded. Smaller states didn't want to join a union with the other ones if it meant that they would be politically powerless and the larger states wanted to have their majority mean something.
There were other compromises too but they were less popular in retrospect.

Commonwealth Care pays the total cost of health insurance for qualified individuals who have income at or
below 150 % of the federal poverty level (FPL) and helps pay for the cost of insurance for other qualified
individuals.
An individual may be eligible for Commonwealth Care if their family’s annual income is at or below 300% of
the Federal Poverty Level (FPL).
you get "free" state health care for people at or below the federal poverty level (i think maybe a bit above too) but it forces everybody else to buy into private insurers (and that is a Bad Thing)
