I know it sounds obvious, but it'd be great if the OP mentioned you need to install L4D2 add-on support from the Tools list. I didn't and everything was just big error signs all around; L4D1 doesn't have a separate add-on thing to install, so I didn't think to check first.

I just think that core balance between survivors/infected is better in realism vs. It's hard to explain why, but you always feel as infected that you have more of a chance to disrupt them and as survivors teamwork is more important. Like I was thinking about it, but the smoker, jokey and boomer are all more valuable in realism vs. than they are normally. Being able to drag someone out of view and other survivors not being able to trivially track them down by brightly coloured auras feels really appropriate.
I'd say it's a combination of several factors. The Smoker and Jockey are no longer mostly useless outside of select spots, hordes are more dangerous, Witches are now actually something to be feared, item spam is indirectly reduced, and teamwork is even more highly rewarded now.
The key difference between regular Versus and Realism Versus is that the Survivors are penalized more heavily for making mistakes. It's not that it's easier for the Infected team, but rather that the myriad bonuses that the Survivors get are offset more evenly by what the Infected are capable of. A more highly skilled team is still going to win handily whether or not it's Realism mode, but you're far less likely to see lousy teams still make it to the safe room only by virtue of being showered in health packs by a benevolent Director.

There's also a strange lesser amount of RageQuits in Realism Versus. This effort is much grander than if you were playing Survivors. Because of this, players who are new or are playing casually a hurt because they aren't helping their team out since one bad infected player makes things extremely more difficult.
This is different in Realism Versus, the Infected have less of a severe effort to put out, and the Survivors now have a equal amount of effort needed. It balances out the difficulty needed to play the infected so people are more likely NOT to ragequit because they feel they can still come back and do bigger damage as the infected. It's no longer 'my team sucks so we're screwed and we aren't going to do anything as infected". It's more "We're going to get those assholes, all we need is one boom and one mistake from the survivors.." which is what I believe Valve originally intended Versus to be like according to the commentary.
Realism Versus is equally a challenge for both teams, not just the infected like the original versus. Not to mention playing as the Survivors rewards Teamwork and communication.

this mutation makes the passing finale much easier
you just have the person with the gnome stand by the old survivors; all the zombies run to him and get killed by the bots, and everyone else is free to grab cans

Well that was kinda fun. I played with my three flatmates on Dead Center on our local server.
What was more fun was deliberately 'dropping' Gnome Chomsky or otherwise causing him to become inaccessible.
* Dropped off a balcony
* Pipe-bombed in the lift (ended up outside it)
* Dropped onto the inaccessible half of the safe room you start in at the finale.
