My favorite accessory I've had was like a Pelican brand rumble pak for the Nintendo 64 that included a slot for a memory card, so that you could use both. With it all put together in the back of the controller, it was this huge unwieldy mech claw that you could bludgeon someone with.

I am sad that online gaming has brought about the death of the Gameshark. I loved finding weird stuff you could do by playing around with it.
I always wondered- how did the Gameshark, Game Genie et al even work? Did they basically hack the game for you? What did they, you know, do? (never owned one)

Hex editing. Part of the code you entered into the gameshark/genie would be what part of the game's code you want to edit, and another part would be what you want to change the value to.
This is an oversimplification, but roughly how it works.
Out of nowhere Madcatz went from the top of the shitty but cheap accessory pile to creating the best commercially available arcade sticks. Crazy I know but true. Only self mods are better these days.
Also as most of you will see in just a few months the Classic Controller Pro is the greatest add on pad ever. So damn comfortable. I wouldn't rail so hard against the DS3's archaic structure if it was as comfortable as this thing.

They accessed the RAM, or intercepted commands from the game, and replaced them with a value or command that would do something that you would want to happen, such as spawn a BFG9000, or make it so that your health won't drop below a number. If you randomly put values into the cheat devices, you can cause all kinds of unwanted effects, such as keeping your character from rendering, or just causing the game to fall apart.
These grew out of a command on the old 8-bit computers called POKE, which let you change values in the system's RAM, to adjust your status in the game. The Action Replay even started as a cart for the C64 that automatically POKE'd some values in RAM for you

Oh, my brother has a Pocketstation. The Chocobo game was pretty sweet, but the batteries died and we never got around to replacing them. Still use it as a memory card, though.

Beg to differ. Hori still makes as good, if not better ones. They are just hard to find outside of Japan. Madcatz is now up there, and I love my TE's, but the HRAP3 SA, and the HRAP IV, are good, to say nothing of the viewlix special version they have.
Madcatz did one thing Horid did not, make it global.

I bought it for Descent II, and it was just about the coolest thing ever. Hard to learn how to use, but once you trained your hands it was super natural.
Don't know why I keep it.. nostalgia I guess. Doesn't work under any modern OS, connects to a serial port which means you can't even plug it in to a lot of modern motherboards. You can get usb->serial adapters, but still doesn't solve the driver problem.
It did not. However, if anyone is interested in the FF8 chocobo pocketstation minigame, it is available freely and officially on the internet (PC version, sans capability to interact back to the main game).

My friend bought one of those at PAX last year. He doesn't even own an SNES anymore. He said he bought it just to own one. He got an amazing number of comments and compliments carrying it around all weekend.

Although he may not have an SNES right now, I hope he got the receiver to go with the gun, otherwise it's useless aside from those PAX-like moments.
And then it broke. And then it broke in a manner I couldn't jury-rig fix. And I was heart-broken.
So, I moved onto HOTAS. But I still wanted force feedback. So I got a Logitech G940.
The setup feels tough, durable, and stable. It's expensive, but it's the only force feedback HOTAS, and it's excellent in its own regard. I for one really enjoy the feel of the stick in particular (I would have bought it by itself).
I think I couldn't go back to foot pedals. That might make me a bad person, but now, it's like giving up TrackIR.
I'd say right now, my Harmony 620 is my favorite accessory.
Wanna play 360? 1 button, BAM, TV input is on, changes to HDMI 2, receiver is set up, and 360 powers on. Same for PS3 (stupid adapter necessary).
Only downside I've found to the Harmony so far is the 'Off' button. My kid occassionally gets her hands on the remote, and if the last thing I did was my Xbox 360 setup, the 'Off' button kind of... turns off everything it had turned on.

I played with one of these at PAX '05:
You wear the same basic goggle setup inside the giant hamster ball, and it tracks when you turn and when you walk.
Most disorienting piece of crap I'd ever experienced, but I'd still pay to do it once in awhile if it was at the local arcade.
Minus the D-Pad, best controller on the market.
