I am now 100% certain that Ace killed Snake and Lotus. The Clover thing was a shocker and I'm still none the wiser what the hell happened with the sub ending. Though murder weapon implicates Ace.
Running out of obvious story paths but I can see at least one more way to make my choices.
You and June run around various locations looking for Snake before giving up and returning to the central hospital.
Does it impact on the ending at all? Yes/No?
Nope. It's all just color and stuff for each of the respective characters. You might be able to spot a clue or two in there though as to what's going on though.

There's also an implication that June became a stone cold-hearted manipulator because of what happened to her, and she was just using Junpei to further her goals for revenge, hence her ditching his ass at the end.
The creators imply that in the answers as well, which makes it all the more eerie.
After being annoyed at getting a repeat ending on my fourth play through I decided it was time to tentatively explore the thread for spoilers on how to get the various endings. With the help of that I just got the fourth ending so am now on my way to the true ending.

I was kind of hoping that there would be some logic in the triggers for the endings but it really is just random luck with door choice. What is functionally different between the knife ending and the fourth ending? The point at which they diverge is just where Junpei goes looking for Clover, heading for the 1st class cabin rather than wherever he finds Lotus' body, and I can see no logical reason in my choices that lead to that divergence.
Other bits of dialogue get added in for no reason as well. I'd done the experiment room escape twice before but only this time for no apparent reason do I get Lotus' talk about shared conciousness bullcrap. The same with Santa and the rat experiment story or the story of his sister.
Making clicking on the safe a vital part of the ending is kind of a dick move as well. By a few replays in you've been well trained to skip through the room sections as fast as possible and as the safe doesn't figure in the escape I had skipped past it a few times.
What I did was play the game perfectly normal the first time and got my ending (knife in my case... bleh). Then, hit up a spoiler-free walkthrough and simply worked my way through the rest, starting with the other bad ends and ending with safe, making sure to also avoid the "dud" ending.
Seemed to be the best way of handling things. Skipping through the text and redoing the puzzles still takes some time, and I'd have probably been annoyed at getting repeats.

Actually, all of the endings are pretty logical in terms of how they're acquired. When you get the True End, you'll sort of understand why multiple playthroughs are a requirement in this game.
As for the safe...
Just speaking from personal experience, the first time I went through that room, I noticed on my way out the door that I hadn't done anything with the safe and it sort of stuck out. In all of the other rooms, mysteries like that were all solved. So it wasn't too tough to deduce on repeat times that I would need to take notice of the safe.
But I can see how other people might be more annoyed by it. There's a pretty huge hint in the coffin ending that you'll need to fool around with it, though.

I see the point, but her intentions were to expose "Ace" for the rat bastard he actually was, and play on his own paranoia cold-hearted cunning by setting it up so that he'd kill the other people she wanted revenge on. She simply set the pieces on the board. It was the others who just happened to play the game.
I didn't realise how comically massive Lotus's boobs were until she sat down at the computer and it shows you the little art bit. It's like someone stapled two watermelons to her chest!

The way she saw it, June was just putting those people (IE, the guilty party/those who actually did have bombs in their stomachs) in the same position those bastards willingly put children in. As cruel as it was, those bastards got what they deserved.

Starting the true ending now! Sub ending was crazy, but then every ending in this game was batshit insane, so I guess I'm not really spoilering anything
So Im progressing through the story, and while it's unique and pretty interesting, I really hope all this random bullshit actually adds up to something.
