Checked every fragment, checked entire flow chart. Nothing.
Missing fragments:
C-Team 4, 5, 8, and 12 (counting across and down from top)
Q-Team 2 and 5
D-Team 7
Non-decision dead ends/incomplete fragments:
C Suspicion (6/7) -> Q Pop Off -(6/7) > D Door of Truth (3/6) -> 2 locked
1 locked (from D Fire: Blank->Drink)
C Anthropic (5/8)
D Transporter (7/9; "stranded pair")
1 locked (from D Outbreak)
And the long string after the FCFS chain, presumably the Force Quit line.
I'm just barely starting to put enough pieces into place and starting to grasp the bigger picture of what's going on in some of these timelines, and holy shit I'm in love with this game. Well, more in love, I guess.
I'm doing the initial decision game scenes, and I was about to say that I'm amused by the previous protagonists being all cryptic about the events of their respective games.
But then I got to D team's exposition. Sigma has absolutely no patience for subtlety, it seems.

About that...
I didn't get C-Team's execution scene after doing all the paths and I just this second realised it isn't some hidden later thing, the outcome can just turn out either way

D-Execution:
Poor Diana. Phi and Sigma going on about needing to shift right there and she just gets to be really confused before exploding.
Has anybody read the manual? Something strange is going on in it. Not real spoilers, but I will just to be safe.
On the character pages, they have all the characters, and the name of the VA for both english and japanese. Except... for Carlos, Junpei, and Diana, the english name is instead "Morphogenetic Field Voice #1/2/3.
I have no idea if that's some deeper meaning that'll pay of, or just a weird way to not credit somebody who doesn't/can't be credited.
"The sausage of Green Earth explodes with flavor like the cannon of culinary delight."
Aka "Biohazard" because it's the reality that leads directly to VLR.
Had to jump directly to a ! on the flow that wasn't obvious. (The fragment might have been "new" and I missed it, though.)
After D-team sets the bomb, and the girls run away.
EDIT:
And there are still two nodes on the chart after it...?
Spoilers concerning something you should know from VLR regarding this ending:
Remember, Akane and Junpei are alive in the timeline that leads to VLR.
It's amusing how VLR's timeline isn't the inevitable disaster that they have to prevent that they present it as, but just something that they happened to be unlucky enough to get.

There isn't a manual on the Vita version, unless you mean digital, in which case I should go check.

Yup. The only outstanding Decision Games I have access to are the ones I listed. I've tried everything I can think of short of looking up an actual walkthrough.

Digital, yeah.
Dunno about Vita, but 3DS version can check that from the home menu.
So I'm cleaning up some loose ends trying to find anything that lets me unlock new fragments, and I think I might have come across a minor plot hole that bugs me (which quite possibly could be explained later).
Losing the decision-game at the end of C-team control room puzzle.
So, after C-team dies because they failed the easiest challenge in the game, cut to: Sigma and Diana escape. Neither of them have, as far as I've seen in this timeline, even encountered Radical 6, let alone been infected. GAME OVER.
Sure it's sad that other people died, but Sigma can tell everyone "Hey folks there's a world ending virus down at this location and I can confirm it by telling you who wins the next 3 superbowls so just fucking trust me and set up a quarantine zone and stay out of it, thanks" and it kinda seems like a bittersweet ending that accomplishes the goal that the major protagonists wanted to accomplish, at the cost of killing some other characters you really like in order to alter the timeline.
Kinda seems undignified to just end that on a "Game Over".

I'm bouncing in order from 'locked endings' to different Teams, which means things are... uh, happening, so far.
I just have a small gripe, something feels a bit predictable:
"Team solves puzzle, then TRAGEDY!" But I have faith Uchikoshi can't fall victim to that for long.
Doing some poking around to see if I can find a solution to my problem, I notice that I only have two of the three "execution" icons lit up on the flow chart. Which is weird, because I know for a fact that I did all the votes, but I think I need to correct this somehow.
EDIT: Yeah, this seems to be the problem. Weird. I thought I was being asked if I had completed the fragments a couple of posts up.

I just solved this problem myself about 30 minutes ago! If you want to know exactly how to do it (sorry, don't know how to phrase any vague hints about it):
Apparently (and I wouldn't be surprised if this comes into play in the future), traveling laterally through time doesn't change decisions other people make. So, for example, to get the D-team execution scene, I had to make Q and Carlos vote for D-team directly after one another. As in, I went back to Q team, voted for D-team, and nothing happened, Probably because last time I played this "level" of the time tree, I had C-team vote for Q-team, to get that execution ending. I then had to switch back to the C-team vote and change their vote to D-team again in order to get the scene to trigger.
it seems like they went full dumb on the story to make up for the fact that it's graphix are bad
i love it
i guess spoilers but honestly i am not sure how you talk about this game
maybe it was just because of the first fragment i chose, but the game instantly got inside of it's own ass in an amazing way. i love it.

This game is thoroughly glorious in the same ways VLR was, you are in for a treat.
EDIT: Also, luckily you very quickly grow used to the graphics. By the end of it I can say I actually significantly prefer these to VLR's graphics.
Gah. this is very much a write-only thread for me; anyway, D-team because Sigma and Phi are the best, Healing room -- minor spoilers and only about that room because it's the only one I've done so far:
Solved the puzzle, didn't press the button, a long movie played.
Went back, did press the button, a _much_ shorter movie played.
I am now all in on this game, because that was _exactly_ what I wanted out of it..

I suspect that seeing bad things happen directly triggers them being shown elsewhere, even if it doesn't actually change anything beyond that.
The eyes, though.
