So, I'm filling out my passport application and I get to this page...
Under my Mother's section, where it says Last (Maiden) are they specifically looking for her Maiden name or do they want her last or either/or?
Also, the DOBs and Place Of Births are optional, but should I make an effort to get those? Will that help anything?

I'd fill out the DOB/POB if you could, because — unless you aren't in a hurry — why risk there being a hold up in the application?
Also, if you are cheap like me and want to save a few bucks, go to a drugstore and get the passport pics taken, they're half the price.
It didn't hold mine up, anyway.
