I might have to puss out and drop the difficulty to Normal for this one...
It's not as difficult as it seems, just mostly requires patience.
DA Wiki's article is rough around the edges, but there's good advice in there. The key is to avoid him while dealing ranged damage/debuffs, waiting for him to become vulnerable before you attack up close. If it's taking forever to wear him down, you're doing it right.

The rivalry with Carver is well written. Unlike the relationship with Bethany, which is piss boring.

I was disappointed to find that the Mage "Hood of the Champion" looked identical to the first hood I picked up in Act 1. Meanwhile, the Rogue and Warrior sets both have unique headgear. What gives?

Fuck the throne room. The party AI went goddamn crazy in there. It would have been an easy fight had Varric and Merril not constantly wanted to know what was at the top of the stares. Guess what. Its the Arishok.
It also didn't help that the game crashed when I was doing well.
In my case, Isabella goes down early like the whore she is due to the initial form's melee AoE (though according to the wiki I can avoid this by moving her around to his rear). I can get him into Shield form easy enough without her, but at that point it's all I can do to keep my tank (Fenris) from getting splatted. I can dip-dive-dodge for a while, but eventually the bastard catches me slipping and does terrible terrible things to my ranged DPSers ( Varris / MageHawke) via cannon ball rush followed flail beatings.
It's been a couple of days since I gave it a go, maybe the break will make it "click" and I'll take him down. Then again, the DA wiki mentions that this is easily a 45 minute slug fest on Hard, and that just doesn't sound like a fun time...
So I have a sealed copy of this on my desk for the Xbox360 that a friend bought for me because i did some work for him. Is there any reason i should return it for the PS3 version?

Would that I could transfer the Helm of the Fallen's appearance to the warrior's champion set. I've always had a thing for impassively badass helmets, ever since George Lucas defined (and retrospectively ruined) my childhood with Boba Fett.
But then I got the helm of the champion. And goddamn it, within thirty seconds I was Options>Game Settings> Hide Helm.
What a missed opportunity.
Here's hoping they give some kind of Templar armour DLC - I love the look of the more senior members' winged helmets.

The cannon-ball is easy to avoid. Just make sure everyone is on constant Hold Position, and pause as soon as he goes cannon-ball so you can micromanage your characters. Keep the ranged characters on the opposite side of the room, while dancing around the boss with Fenris. Isabella is probably best kept to the side until he spawns his trash mobs, then have her go to town on the boss.
45 minutes is probably a bit extreme. Maybe 20ish with 2 ranged? My last kill was less than 15 I believe, but it was my second time killing him and I had a 3 ranged 1 tank party.
DAMMIT ALL, I GET TO THE SAME POINT AND IT STILL HAPPENS! WHY ARE ALL THE ENEMIES MAKING THE SOUND EFFECTS OF GIANT SPIDERS!?
EDIT, I mean, I know some actually spawn at that point, but then they mess up the sound effects for the bandits and everything else, too. Ugh, so many damn bugs with this game....
EDIT 2: Wait... I wasn't messing up the sound effects with Varric's cross bow or something, was I.... I loaded to a previous save, and I heard the sound effect when fighting some Qunari at Wounded Coast.... hmmm...
You know, for all the complaints about DAO being generic fantasy, even without the whole continual reuse of the same areas thing, 90% of the areas in DA2 are just so....bland.

Ugh, and now Merrill's armor bugged out. Lost both her rune slots too. Has anyone else seen this happen with companion armor changes? I've been googling and reading the official forums, and haven't seen it really mentioned.

So I respecced my 2H Warrior Hawke to a Berserker build (22 willpower atm, level 13). Only 2H talents are mighty blow + its 2 upgrades, Giant's Reach, and the Sunder. Got Berserk, -2 stam on hit upgrade for Berserk, and Barrage from that tree. Got most of Vanguard (up through the last talent, plus Cleave + Claymore + damage resistance reduction talent). Have 2 points in Battlemaster for Bolster and the +10 stam regen talent
it is so fun
You mostly just auto-attack but you do so much damage if you can keep your stamina high. Cleave + Barrage kills groups of people ridiculously quickly and with the Vanguard end talent you get tons of killing blows so you have tons of stamina. Fully upgraded Mighty Blow is there for combos with Petrify.
I can't wait til I can finish it with the additional Berserker talents + Battlemaster talents. Then maybe grab Scythe + Whirlwind at higher levels.

Yep. For whatever reason apparently getting certain updates before other updates causes rune slots to disappear. Very irritating.
I absolutely love the ability to have your party members chime in on certain situations. Seriously, it's great, and needs to be in more games. So far my favorite is
Varric convincing that slaver that the random dude he kidnapped is really the viscount's son so he better let him go.
The best Varric interaction is in the Deep Roads and completely unmissable.
When you have him one shot the Hunger Demon with Bianca when it tries to make you leave all the delicious treasure after the Rock Wraith.
Thats missable if you kill the demon when you first meet it.

45 minutes is probably a bit extreme. Maybe 20ish with 2 ranged? My last kill was less than 15 I believe, but it was my second time killing him and I had a 3 ranged 1 tank party.

it is so fun
