Yes, it uses big characters for the sides of the face that span multiple lines while still just being a single character. The mouth is also a real smiley not just a dash

When used in some places it causes formatting to get thrown off and people are like "O SHIT"

I've seen the large characters you're talking about. Dickheads use them on craigslist all the time to make their ads stand out. Something along the lines of this:

Only with lines that go up an additional line.
