So, an argument just broke out between a few of us in the movie etiquette thread and I thought we should just hash all this out right now.
First off, you have your national/international fast food chains. I'm talking from an East Coast/New England (northeastern) American perspective. My choices are generally Burger King, McDonald's and Wendy's. There's also a Checkers here. NYC itself has a few extra options, like Five Guys and White Castle. It doesn't have an In-N-Out Burger, which I consider the crème de la crème of patty-based fast foods.
Personally I'd rate the ones I have easy access to as follows: Five Guys > Wendy's ~= Checkers > Burger King > McDonald's > being hit in the face with a shovel covered in Ebola virus > White Castle.
If I were adding In-N-Out Burger to the mix, I would place that WAY above the others. I'm rather upset that there are none on the east coast.
There are some people here who think Five Guys is better than In-N-Out Burger and I think that is utter silly goosery. First off, while price doesn't really affect taste, Five Guys is expensive. A burger is $6.99 with
nothing else. Just the burger and condiments (admittedly prepared any way you like, which is a service I enjoy). A double-pattied burger is $8.99. I can go to Applebees and get a cheaper burger. Probably not one as good, but still. I think $9 is reaching beyond normal fast food pricing. Then you add on fries and a drink and you're looking at a $15 meal.
In-N-Out is the perfect blend of meat, bread, and cheese, and it is relatively inexpensive. Like McDonald's pricing for what tastes like an orgasm.
That's my stance, anyway.
Then you get beyond meats into chicken and whatnot.
For instance, Popeye's versus KFC. KFC isn't bad, but it's also not good. Popeye's is much better. I'm not too familiar with other chicken-related fast food places, but in the East, these are the big two I am familiar with. Popeye's makes the perfect butter-drenched biscuits which just taste amazing and also fatty but you are distracted by the amazingness of it.
Then you get into non-chicken, non-burger type fast food. For instance, Taco Bell. Taco Bell, like White Castle, is gross. I only go there when I'm drunk. Taco Bell is like the one night stand of fast food places. You'd never go there sober, and then you wake up in the morning to a few empty wrappers lying in your bed thinking "oh god, what have I done?" But it's the most common "mexican fast food" place here, I guess. Chipotle is much better. I also like Qdoba and I think the general consensus is that Qdoba > Chipotle, but Chipotle is more common and I swear their burritos are like 2 pounds each.
I guess that's all I have to say for now.

these are the superior ones, everything else is bad!
Five Guys is good, but like you said, the price makes it an every once and a while kind of deal. I do enjoy, however, that they offer jalapeños as a condiment for my double bacon-cheese burger.
Meh, had Jimmy John's free sample sandwhich once. I was unimpressed. In my opinion, a creative 3-year-old could have produced a better sandwich with sand from a sand box.
Jimmy John's main advantage is that in many areas, they are the only place that is open past 9 pm that serves something other than hamburgers/tacos.
Yeah...never actually ate there, wasn't sure if it was really fast food or what.

Where I went to school it was 4.99ish for a burger, which is the normal burger size (plus any combo of toppings and condiments, including bacon)
In LA it is about $5.39.

Hmm, sounds like there is an opportunity here to provide a better service. I like that the area where I'm located, has multiple food types represented in a late-night fashion, so no matter what kind of hungry-drunk you are; there's something for you to eat (cue in, "that's what she said" jokes).

And that is basically all the fast food I will eat now.
Where I'm at, a double-patty with a regular fry costs ~$11. Prices may vary by franchise or region, but this is speculation on my part, not researched fact.
It's all in what you get. There are seriously some quick and cost efficient gems there. It's basically like mine sweeper on expert mode.

I don't know about that. I think they cook everything in a vat of disease. I have never eaten White Castle Taco Bell and not felt sick the next day.

And I went by my first White Castle when I was visiting Cincinnati with my fiance about a year ago. I asked her if I could go there, having never gone before, she said "No, you don't want to do that, trust me" and that was that.

Once you're into those price ranges, you might as well go to a restaurant. Islands Burgers is better than In-N-Out, and you can call-ahead for carry out, but they're restaurant prices. Once you clear 10 dollars a meal, you open yourself to all sorts of competition from local stores.
Case in point: KFC. We have an amazing restaurant that does pick-up near my house that sells 4pc Wing, Breast, Thigh, and Drumstick for 7.50. That comes with home made cornbread and two heaping portions of sides. Aside from the fact that it's so delicious to die for, and the chicken pieces are roughly twice as large as at KFC (and made fresh), it's cheaper. KFC charges 6.50 for the same cuts of chicken, with no sides and no cornbread. Upping it to a combo meal takes you to the 10 dollar range. Also, if you decide you want to sit inside, you're treated to a local place people actually want to hang out at, musicians on the weekends, and respectful high class clientele. Versus, you know, KFC where they have to put the fluorescent lights on ultra-brite to keep away the meth heads.
Ironically enough, this is also the case in Eastern Washington due to the large migrant population. Fucking fantastic Mexican food.

I think NC just sucks for late night stuff. Where I went to college, even the bars shut down at 2 or 3 am on weekends. And the only places open 24 hours were a pharmacy, a grocery store, Cookout, and Taco Bell. Hell, even Wendy's closed at 1 am.
Where I am now, the only places open 24 hours are a Wal-Mart, McDonald's, and...I think that's it.
That's... unfortunate. On the other hand, McD's isn't terrible food for when you're drunk-hungry. However, if you're a third-shift worker and want something decent, that sounds like balls.
There seems to be an upsurge in a little more expensive, not so fast, burger places tood. We've got some Fatburger locations here which is pretty good, as well as Vera's Burger Shack which is awesome, but certainly not fast.
But really, if I'm in a mall, I'm probably eating Vietnamese from Vina.
Pho King. Yes.. it's a played out joke, but earnestly there used to be a restaurant in our area with the name. Decent pho.
I find there's usually at least one item I can find in a fast food place that I enjoy eating. Generally that'll be the only thing I order.
Taco bell: crunchwrap supreme
Taco time: soft super taco (which in my day was called a super soft taco.)
McDonalds: chicken nuggets (till they discontinued the hot mustard sauce in Canada,) egg mcmuffins or fries.
Burger King: whopper (with extra mayo and ketchup, because the thing is not nearly as sloppy and delicious as it was ten years ago.)
Wendy's:spicy chicken sandwich or the chili. Used to also include the junior bacon cheeseburgers but they downgraded those, too.
A&W: Bacon and Egger, mozza burger.
There are no particularly special fast food places anywhere near me. I remember being excited on a trip to the states to finally try Jack in the Box (which is totally disgusting, by the way. I assume their 'two taco meal' is the worst thing on the entire menu.) I do hold a special place in my heart for the deliciously greasy burgers at burger baron, along with their french fries (which are underseasoned, but that's a pretty easy thing to fix with french fries) and deep fried mushrooms.
edit: ooh, I forgot: Edo of Japan. That may very well be the best fast food thing ever, though I wish they had drive-through windows.
