Fistol healing is a great way to get started in nightmares. The key is Empowerment and high enough crit rating. Have your three or four (Nuture, Surgical Steel, Cauterize, Creature Comfort) hots always rolling on the tank, with crit rating proccing Empowerment. You can heal the first three NMs in elite QL10 blues. There are other passives that help a ton too. Making Amends, Healing Sparks, Brawler, Shadow Medic, Nuturing Gift to name a few.
Cleanse isn't the only advantage of Fist/Pistol. Backup Drone is pretty much one of the purest expressions of healing love there is in the game.
P.S: Deadly Aim is a pretty multifunctional buff as well. Boosts healing, boosts damage. No buff lockout, so the more Deadly aim the better.

It seems to me that Diamond Grit functions the same way. Barriers being effective for single hits over their limit would make them the obvious counter to damage spikes.

I am really liking this game since I picked it up. Not much in the way of social stuff though, it feels very "single player" so far. Guild wars 2 i think beats this game in the social aspects so far.
Well. Mostly it's because pretty much everything is possible to do solo and everyone can build into a solo build. So that's kind of something that doesn't really encourage teaming up.
On the other hand there is nothing that's stopping you from socializing either. You pretty much have every tool necessary if you want to, including nice hangouts and the ability to open up separate chatwindows only for socializing (so that it doesn't disappear in the infostream).
Once you hit nightmare content though it's very very nice to have a relaxed cabal of friends. You know, the type that enjoy running stuff together and just chatting in general. The type that isn't going to go elsewhere when people want to experiment, take it slow, do something different....well, anything that isn't maximized dungeon speedrunning using the One True flavor-of-the-month build.

Also I feel like I need to address the "socialness" of GW2
Getting a quest pop up that says "Oh hay kill all these dudes", and then running around killing dudes while other players ALSO kill those dudes and a meter fills up and everyone gets credit is not any more social than any other MMO.
I ran tons and tons and tons of those such events, including the big events that spawn bosses and such, and most of the time the players are stone fucking silent, and they sure as hell aren't trying to actively help others. Even the classes capable of buffing and healing large groups of people would just be using DPS weapons and trying to DPS stuff as fast as possible.
Though I think it is fair to say that TSW isn't very social and I agree with your "social" assessment of GW2, but at the same what MMO these days is?
That being said, I certainly wouldn't mind seeing more GW2 styled Public Quests pop up in MMOs, but I also wouldn't want those to be the only thing in the game (outside of Dungeons and Story quests of course).
As an example of what I mean, one of the very first things you end up doing in Kingsmouth once you get to the Sheriff's office is defending the walls from zombie hordes. That right there could of been made in to a GW2 styled Public Quest. Hell, every time I find myself at the Sheriff's office and I hear the sirens go off I always help out, mainly because it is fun. :P

To me saying that a modern MMO isn't very social is like saying you need to complete quests to get XP. In my mind that is pretty much par for the course. I'm hard pressed to think of a single MMO that has been released since WoW where I can say there is in fact a lot of socializing and community to it (outside of Raiding). EVE Online I suppose fits the bill, but that is kind of a special case.

I had a rough time with that mission when I was starting Transylvania
It is just a really fucking hard mission to solo
I got to tier 4 of 6 and then I died and had to go repair, when I came back all the guys I killed respawned I am now however blowing through Egypt. I actually think it is easier then Blue Mountain.
Of course people aren't kitted out for buffs and such when soloing, but people absolutely do actively help others in GW2. The "socialness" of GW2 is not about talking (not needing to talk to cooperate is kind of the point), it is about not actively resenting the presence of other players. TSW was smart enough to mitigate this to some degree by allowing many of the mission completion criteria to be shared without being in a group, which is definitely a good decision on the developers' part. However, at best, I am generally indifferent to the presence of other players around me in TSW. Contrast that to GW2 where I am glad to see other players, as we share goals and rewards and we can revive each other if necessary. In TSW there is no revive so even if I try to help someone who is in danger (with no material gain for me), I probably won't be able to save them a tedious corpse run.
Even if they don't do public quests and such in TSW, I definitely think the game would benefit from loot/XP sharing and a player revive.
Edit: On a completely different topic, I thought I'd share a mod I have picked up: CustomUI. It lets you move/resize the UI elements as you please. Someone in PA chat asked for this sort of thing and I have found it to be quite a well designed solution. Example in spoiler:

What settings do you have it running at? Because we've got almost the same machine, but yours is better (3.2 quad-core/4gb ram/1gb radeon hd 4890), and my performance isn't that bad. At Low/No Anti-aliasing/1080p it usually hovers around 40-50 when running around/soloing and dips to 15-20 when a lot of stuff is on-screen.
What are the requirements for nightmare? Aside from killing the gatekeeper of course.
You have to finish all eight Elite dungeons to get access to the Gatekeeper, and you probably want to be in mostly QL10 blue stuff when you fight the Gatekeeper

Had a guildmate with a similiar problem. She dropped her screen resolution down one notch, and her average FPS shot up 30-40 frames.
Some of these solo instances really kick my ass. I was doing pretty decently in Egypt and then I get a call for a promotion mission and I can't complete the last tier. They want me to fight against three enemies, one of which can't be harmed unless you use the environment and the other two are pretty hale on their own.
Bah.
Mostly just huntin' monsters.

It will keep track of damage, target healing and self-healing, as well as the percentage of crits, pens, evades, glances and blocks. You can even open a secondary window that logs each event in real time.
Will have to check out these crafting and meter addons. Think I've firmly moved into the 2nd part of Egypt now. I definitely don't like temple as much as the first in terms of navigation. Upside is that I'm almost flush with QL8s and a pair of QL8 blue weapons, so I should be ready to move into the final stretch (well, final green stretch). Experimented with a focused fist healing regiment using backup drone and the crit buff, and it seems both solid and very easy for me to control and operate. I'm eager to try it on the next normal dungeon.
Are any templar Cabal officers checking this thread regularly? Tried to get an invite this weekend, but I may not be playing late enough in the evening to hit a primetime zone.
To update on Funcom news, following the press release last week which some people apparently misread, the layoffs are pretty significant and look like they may hit Montreal, which we know is at least some of the TSW staff.
The 110 employees of the Funcom’s Montreal studios will know early next week whether they still have a job.
The struggling Norway-based video-game developer announced Thursday a major restructuring.
The Montreal studio’s chief has resigned.
Already hit by layoffs in August, staff were glumfaced at the company’s downtown offices Friday.
Many more had been given the day off to deal with the bad news, delivered in person by the firm’s CEO from Oslo.
“Hard times, hard times,” one staffer said as he left the building on Ste. Catherine St. just west of Bishop.
“They’ve started talking to people individually, but we don’t have any numbers yet,” said another.
“We’ll find out Monday, maybe Tuesday,” said a third.
This is really unfortunate, and I hope any of the staff that get hit find places elsewhere. Some of the cool design changes that TSW introduced, and just the general creativity of the game, definitely needs to make it into other MMO's. I hate seeing a studio punished for trying something new. Especially when it was as cool as TSW, even if a bit niche.
TBH I don't think TSW is so much being punished for trying something new. It's more that it's a subscription MMO in a year with several other MMO's some of which with no subs meant it never really got the attention it deserved.
The fact that to me the only fault the game really has is I wish the skill system was even MORE open (lower AP costs, much lower skill point costs) to allow me to have more fun fucking around mid game implies it's doing something very right.
My issue isn't that it's too grindy but more that the grind in this case leaves you with a limited set of skills.
In a traditional MMO, say I was playing a monk:
By level 20 I'd have tools that let me both act as a healer and a DPS.
In TSW's AP system you have a case where you want to continue leveling up one weapon and one tree from that weapon. For example I'm going to be going after Paladin for more or less 400 AP (if not more). During that time period not only do I not get to switch it up (because all my 'best' skills are in sword/pistol) but I'm only good as a DPS.
It's just one of the downfalls of the system they picked out. I struggle to really think of a way to alleviate it as well.
I wonder how much of an impact this will have on Funcom's contiued support of the game.
Edit: albino bunny: agreed. The character development model as it is kind of feels a little grindy for a (now) B2P MMO.

They are SUPPOSED to fight each other at the start, but they all three just aggroed me everytime I tried for an hour.
My talismans are a weird mix of heal rating and straight health, but I couldnt get a good combination that kept me alive long enough until I finally just cheesed it with Line of Sight abuse, impair abuse, and chugging a bunch of QL10 buffs I got from the mayan event.
A little explanation that "hey these shrines stop his stupid attack" would have been good. You get little hints for all the dungeon bosses, why not this motherfucker?
Really the biggest danger they face is not being able to put out enough content with the smaller teams to make enough money eventually draining out the game and causing a shutdown.
That wouldn't happen overnight though and is not an around the corner type of scenario.

Kay's tip is #1 to pick up on. I'll throw in another tip- the statues on either side of the area you fight in are not just for show.
I managed to do this after a few tries, using the tips above, and having some health potions at hand (used one or two at the time I finally beat it).
I think I'm going to pause the mission and work on my equipment/decks a bit before going back to the fight.

That mission beat me first time, then I paid attention and kicked its ass. Also, to those talking about having tank and heal talismans, you NEED DPS talismans. @Arch - try to be split between the three for solo. You only need enough tank to survive burst. The DPS and healing is more important.
