Without jungler intervention, Malz is probably going to win his lane. He's a very strong duelist, and a very strong pusher, and there aren't many champions who can reliably beat him 1v1. Really mobile champs like Ahri and LeBlanc can do pretty well by making it hard for him to land his full combo, and rushing a Quicksilver Sash is great against his ult. The new Banshee's Veil should be a pretty good MR choice against him, as well, since it boosts regen and most of his damage is over time.
There are two main ways to win against him. The first is to call your jungler for a gank; even post-6, if either of you have any kind of hard CC (and you probably should), then his ult can be interrupted. Since he pushes really hard and has no escapes, a jungler who's paying attention should be able to get some decent ganks against him.
The second is to give up mid as lost and help other lanes. Malz's roaming isn't great because he's fairly slow and has only one hard CC with a really lengthy cooldown, so he won't be able to react well. He might be able to take your middle tower, but if you manage to take a side tower and grab some kills then your side will be ahead. This is riskier, but if all else is failing then you might as well go for it.
In terms of specific champions, Cho's probably not a great idea against him; Malz is the tank killing mage, and he'll rip through Cho's bonus health. Assassins that can interrupt his ult are the best bet - I've had some Dianas manage to time their E so that it knocks me out of my ult, which was an unpleasant surprise.
Edit: Roz's suggestion of Fizz is a good one - if he lands a shark, then Malz is going to die and essentially can't do anything about it.

Gragas is still a beast. He's not just for disposition. His ult at rank 3 does 450 with a full 1.0 ap ratio in an AOE. You can steal baron with it from a level 18 smite if you have enough AP. He's good for getting picks with his barrel if you catch people out, but even if you don't it's still bonkers damage, and his Q has a 0.9 ratio. All around his damage is very good.
Annie can go tanky with ROA/Rylais/Liandry's and be a beast, or she can go Deathcap/void/zhonya and just burst. Neither one is wrong, it just depends on team comp, and how you want to play it.
Diana can beat Malzahar mid. If you can land Qs on him to harass by the time you're both 6 you can just all in him. His ult at level 6 doesn't do that much damage and your shield mitigates a lot of his damage. Just wait until he uses E or Q on the creeps then go on him so he can't hit you with all 4 spells at once, but even if he does you'll probably force his flash and live. It seems like whenever I play mid (typically Diana or Lux) he pushes me under my tower, denies me farm, and just generally wins the lane ultra-hard. If the jungle comes to gank Malz ults and kills one of us so he's still getting ahead even if we kill him in return.
I get that neither Diana nor Lux are particularly great picks against him, but I have trouble figuring out who a better pick would be. Maybe Cho.
Here are some tips besides getting your jungler to harass every time Malz overextends. I usually play Malz mid and most often Anivia is my bane. Basically if you can wave clear at the same speed or faster, Malz can't leave the lane either.
That said, another viable way to shut Malz down is to make him waste Malefic Visions without taking damage yourself. No Malefic Visions jumping from one minion to the next = no mana for Malz. Zyra is perfect for that with her plants. Harass when he casts Malefic Visions. He doesn't have any options to counter when it is down besides Call of the Void, and that drains his mana.
Post 6, another way to counter Malz is to interrupt his combo/ult. Delayed stuns/silences are awful to play against as Malz. This includes Anivia's stun, Kassadin's silence maybe even Gragas' ult. No combo = no kill = sad Malz. You need to know when Malz is able to combo you to death and avoid it at all costs.

There's at least one youtube video of Saint being a total dick to Elementz and not letting it go, knowing that their house is wired and the whole thing was being streamed. Even if we all agree Elementz' work ethic wasn't as high as the rest of the team, after a certain point he should have let it go. Then there's a separate clip of the next morning of Elementz forgiving him. I probably would have done the punching in the face method mentioned above if I was Ele or Dyrus in that situation.

Ya. Its not one barrel, its the combined dmg of both (and maybe the knockback helps?) The dmg from both barrels far exceeds what smite can do.

Mpen works on creeps too and you can time it to pop at the same time. Lux can almost do the same with her ult and e.

Why not? It's cheap, easy to build, and has every stat that he wants outside of using his ult (cdr, pen, ad, crit) while also having 4 to 7 seconds of attack speed boost depending on how effective you are at timing your ult.

If you don't ult it still has every stat he wants for team fighting, increases his ability to dash around and proc his passive while also increasing his damage by a massive amount. The fact that you can turn on the bonus as/move and use it during your ult is just icing on the cake.

No it isn't, Sanguine and Bloodthirster are the most realistic gold-efficient items you'd want to buy.
Only if fully stacked, you don't count the armor penetration advantage, the gold advantage from having gp10 in the build order, or the time advantage from completing GB earlier and not having to stack it. Counting GBs active makes sense because you can time it how you want.
The breakpoint is 20 stacks if the enemy has zero armor. So the breakpoint for a target even if they have a lot of armor is going to be higher. It's going to be pretty hard for BT to break its efficiency. On a 60 armor target and assuming some pen from masteries ghostblade is strictly superior. From just applying the bonus damage from pen to the scaling part. If you have whisper then this changes things a bit because the attack speed increases the value of the extra damage but the absolute minimum value for which GB dominates is 80 armor. More likely its 80 armor and increases as you gain more items.

If you have whisper then this changes things a bit because the attack speed increases the value of the extra damage but the absolute minimum value for which GB dominates is 80 armor. More likely its 80 armor and increases as you gain more items.
