It is definitely a personal thing that happens in my brain for a variety of reasons. I could go on whining about it for a while! but I am stopping here

The only time QTEs work is when this game explicitly advertises them and embraces them, such as God Of War or Asuras Wraith or that game where Jason goes missing. Those games sell it upfront.
Hiding them in other types of games or using them to just give the player a reason the pay attention while cool cgi happens is just the worst.
QTEs during gameplay or for certain sections can be fun.
QTEs randomly thrown into cutscenes basically are just going to be a guarantee fail for me because that is when I do things like check my phone for texts.

Kind of. I'm playing through Arkham Origins right now. I've been saving it for this time of the year. There's a pretty great fight with Deathstroke early on that is essentially a QTE, the Arkham style combat and a fighting game all at the same time.
Heavy Rain, which I think used them a bit too often, really satisfied this. I gauged my reaction time and based on that my character would often succeed or fail at running, leaping, fighting, etc. I enjoyed it quite a bit.

The new Zelda looks pretty great, no one believes that No Mans Sky will be what they say it will be but everyone really wants to believe them.
