There really are basically no horror anime I can recommend unconditionally. I do not know if it holds up at all since it was absolutely ages ago when I watched it but I remember Higurashi freaking me out a bit. Mononoke is trippy as hell but I wouldn't call it scary. Same with Mushishi. And those are about the only anime that I would qualify as both "good" and "horror".
Now there' plenty of good anime that will make you feel bad. But that's like existential horror not anything that will like give you palpitations. Like horror is just not something that anime has almost ever done well. And there are startlingly few shows, compared to how much anime is out there, that have even ever tried. Like it's rare to get even one true "we're going for spoops" show a year, and even rarer if it is not terrible. There are spoopy little bits in otherwise good shows, but that's just it, they're just bits.

devilman crybaby is something that is tremendously hard to recommend to any human despite me thinking it is one of the most memorable pieces of media I have ever consumed
it's something I think people should watch, but holy shit is it a lot

also, paranoia agent and mononoke are my horror recommendations
mononoke isn't 'scary' so much as upsetting and confrontational, but it's real good and Medicine Seller is strong and my friend
paranoia agent is deeply upsetting and sometimes scary and incredibly relevant today for many reasons
Also, good horror tends to use its medium to its advantage and loses some of the punch when adapted
the Junji Ito anime doesn't make you physically turn the pages yourself, after all - the events just happen (and since they're animated rather than still snapshots there's less for your mind to invent)
