The Guiding Principles and New Rules document is now in effect.
The only ones that come to mind are Super Mario Bros., Megaman 2, and Ninja Gaiden. (I warped for SMB, though, from 1-2 to 4-1). I could never quite get through Contra on one man.
edit: And my proudest gaming accomplishment will be 5-starring Expert in GH2, just as soon as I do it.

I borrowed Rygar for the NES from a friend of mine.
I had no direction booklet, and I never figured out that you can upgrade your weapons and stuff with all those stars you pick up.
So I beat the game with the default weapon and no upgrades whatsoever.
I bet less than 3% of this forum has ever played Rygar, and I bet less than 1% remembers the game enough to appreciate how hard this was to do.
I had a lot of accompishments (server/global firsts) with my rogue back in classic/kunark EQ days, but that was also a very long time ago.

I am part of that 1%. Color me impressed.
Also, Sonic 1 and 2. Forgot about those.
And if we're counting RPGs, I've beaten most of those without dying. But really, c'mon.

Contra.
NES Contra.
Would just beat it, over and over again...
You're telling me, you can play the original Contra, without dying? And you're not kidding?
Once you know the bullet patterns and the enemies, it's not that hard.
I always had the most trouble at the boss of the ice field stage though, the jumping dude with the sliding flootballs.
My perfect game efforts would always end there and only there.
Once upon a time, I could beat Bionic Commando on one life. I think I beat it a couple times without even getting hit. I got so good at that game, I could beat most levels touching the ground an absolute minimum. (I seem to recall there was one level you could beat without touching the ground at all, but it has been so long (10+ years) that I could be misremembering.)

I used to be able to do this too.
Well that's cool, but no I never knew anyone personally that could do this. I figured it was possible and all, not just as easy as you make it out to be lol.

I used to love sitting down and just tearing through that game. I don't think I ever tried it without hitting the ground, though. Probably not, since I always parked at that place down the big mineshaft near the beginning that had unlimited guys thrown at you so I'd fill up my health bar nice and early.
RE1,2,3,4 I didn't bother much with the other RE titles
Silent Hill
Zelda Link to the Past
Wind Waker
Diablo 2
Medal of Honor
Super Mario Brothers, and SMB3
That is all I can think of off the top of my head

I have none to list. I suck.

As mentioned, it really wasn't that hard once you knew the game really well since everything followed really strict patterns.
Yeesh, Contra > Metal Slug. I never understood how friends of mine could play that game without giving up ten minutes later and curling up into a howling ball of rage. Least of all how someone of them could play it to a similar "beat with one hand behind their back" attitude...

I had a lot of accompishments (server/global firsts) with my rogue back in classic/kunark EQ days, but that was also a very long time ago.
And I bet about 0.5% of the members of this forum still own Rygar and an 8-bit NES. *raises hand*
Games without dying... SMB, Super Mario 2, Super Mario 3, Jaleco Baseball (it's a joke, remember charging the mound?), Rygar (but with upgrades), Metal Gear: Solid... Viva Pinata? :P
There may have been others, but I've slept and drank beer since then.
Man, I got good at some shit as I kid but I can't say for certain what I've one-lifed.
I've never played the NES version of Rygar but I could trash the Lynx version, which I understand is a port of the arcade version and very unlike what the NES got. Still a haaaard motherfucker. Could also trash Gates of Zendecon, which was pretty easy.
I never owned LTTP but I borrowed / rented it enough to be able to 100% that shit on 1 life without turning the game off. That one is a definite, did it many times.
Shadow Dancer... I would hazard to say I'd 1 lifed that. I had that shit memorized cold. No longer. Same with Castle of Illusion. Which I just rebought, too... need to fire up my genesis. Kid Chameleon... only w/ last boss warp, heh. TJ&E... probably, though chancing a present and getting a total bummer happened pretty often.
I could 1 life SNES SFII & SSFII, dunno what difficulty I got to though. Probably just a notch or two above normal.
edit: I don't recall ever dying in Silent Hill 2 or 3. I could 1life Mario World skipping most of the game but since lives flow like water I usually opted to play it fast & loose.

I bow down. I beat that game back in the day, but doing it without upgrading anything..damn.

Megaman 1, 3, and 4
Megaman X, 2, 3, and 4.
Sonic The Hedgehog 2 and 3
After an initial playthrough I would know all the patterns on all the bosses and stages and just breeze through the games so easily.
I'm pretty sure I've beaten a large number of RPGs without dying.
Final Fantasy 7, 8 and 9 I'm certain. Also Wild Arms.

I beat panzer dragoon zwei in a toys 'r' us display on the first try, without dying. It was kinda one of those things where I was hey, this is kinda cool, hey, is that the ending already?
I also beat forgotten worlds on the genesis without dying.
World heroes and killer instinct two were both one quarter runs, but fighting games don't really count, do they?

He claims with great sincerity that he once beat every game on Mario All-Stars, in sequence, on a single life.

Straight edge run.
I don't think that counts as an accomplishment though.
Also, Poll thread!
Halo 1 & 2 on Heroic.
Can't remember what else.
come on, i was like 10.
If they do, then Zelda LttP and FFT hehe
Jordan Mechner really needs to make more games.
EDIT: Here's my list.
...wait fuck
Getting through an entire level of Metal Slug without losing a life. C'mon, it's not possible to complete those games!
