To be fair, Bloodlines is Horror/Action/RPG.
The little bit about the snuff film in the game was really good too.
Well, then I nominate Ravenholm. Though some say it wasn't scary, it had some real shocking fast zombie moments.

I would have to say, one of the only zork games to really creep me out--
Didn't see it mentioned, but one of my (least) favorite game experiences is the warehouse full of Protocol Droids in System Shock 2. I can remember being perched on a pile of boxes in a complete state of panic, being unable to collect my thoughts because all I could hear were the droids walking around looking for me.

Eternal Darkness is the only horror game I ever managed to play and I only managed to persevere because Pious interacting with his Ancient is pretty awesome (especially Xel'lotath).
However, the Sanity effects never really scared me that much. There were only three that creeped me out:
The sound of some... thing... knocking heavily on a door somewhere, the statue's head turning to watch you walk down the hallway and one of the NPC nurses in the WWI level hissing at you with demonic eyes and very sharp teeth when you try to talk to her (as described in a regular text box which would have had some normal phrase if the bonethief I rescued her from hadn't lowered my sanity so much).

Not exactly a horror game, but I'm playing Breakdown again, and that has some nerve wracking parts. Being chased by the T'Lan before you power up and later being chased by Solus were pretty scary to me.

A friend of mine got rid of Fatal Frame before I could play it HURGLEGHGHGH

Hahahaha, yeah, I honestly had to stop, pause the game, and put the controller down to compose myself again after that. It just catches you out brilliantly.
The mirror part in the hotel didn't get to me because I picked my way through that place carefully, anyway. SWAT 3 & 4 taught me how to clear rooms and deal with surprises. What did freak me out was the first time a meth-head attacked me with the needle. First thing I did after beating the game was replay that level on FPS mode to get some revenge on those skinny fucks.
And the creepiest part of the game for me, oddly enough, was the mannequins. Where every time you look again, there's more of the fucking things and they've been moved around. That was brilliant.
The bear, I knew about in advance, but I'm convinced it wouldn't have changed things much going in cold. There was an awful lot of setup, and then the game autosaves for the first time in a while. Anyone with the slightest bit of gaming instinct will feel their spidey sense tingling, and steel themselves for what's coming.

Completely Unrelated, I just picked up Condemned 2 for $20 for the PS3. Seems pretty good so far, I feel like I missing something by not playing the first one, too bad it never came out for the PS3. What is the consensus about which Condemned game is better/creepier, 1 or 2?
Well, Condemned 1 did come out for the PC too. I don't think you absolutely need to play the first one, but it sure as hell wouldn't hurt. I personally like how the stories connected and the first foreshadowed the second.

I just tried playing the original PS1 Silent Hill. The beginning is pretty creepy and atmospheric. I like the crazy camera angles and all the foreshadowing in blood of what was coming around the next corner.
Clumsy tank controls and firing kind of made it hard to continue, though.
ed: Condemned 2 does sound interesting, I played the PC version way back when. I probably don't remember enough of the story, but that's why God invented Wikipedia. Only 500 points on Goozex, too, if I can figure out how to raise it without giving away all the good games I have left...
Condemned 2 is a better game, the controls are vastly improved and it has some really neat moments, but I think I prefered the story of the first. They're both really good games though, and both do a great job of creating an oppressive, grimy world for you to go poking around in.

which one would you recommend for a Halloween Sleep Over/Party
Umm.. it depends what you're after. The first's story starts off more normal and things get weirder and creepier as it goes on, but combat and gameplay-wise it's not as solid as 2. This has a more versatile combat system (as well as far better animation when beating guys) is considerably more violent, whether that's better or worse is up to you. Story-wise, whereas 1 takes a while to really hit you with the crazy stuff, 2 leads almost straight in with weird and messed-up.
It also has arena stuff where you can just beat the snot out of a bunch of hobos, if you so choose.
What kind of thing are you looking for? One person playing a game while everyone watches and hopefully gets creeped out, or something different to that?

Now, how about some Sanitarium (PC)?
That game wasn't "hard" since you couldn't really die, and it wasn't shock-jump scary, but it was definitely creepy. The pumpkin patch level is just disturbing.
Not passing the buck, but there should be a PC demo of Condemned, and I believe there's a Condemned 2 demo up on Live. Don't know about the second, but the C1 demo is of the first level, and should give you a decent taste for what to expect from that. It's more realistic and 'grounded', whereas C2 takes a more supernatural approach earlier on. They're both pretty high on tension and scares though. C2's combat can be a little trickier, it's improved and they expect you to use those improvements, so if you've got people who don't play many games they may die a lot.
I would possibly suggest leaning towards the first one, if I had to make a call, but if you've got the opportunity to try both I would go with that and see how they feel.
hopefully it'll be scary enough the guy will want to swap out, at which point another person becomes the victim of whatever scare-a-thon we choose

If you're up for a game that's not Condemned 1 or 2, try one of the Fatal Frame games. It is FULL of scares, but they tend to start kinda slowly. That might be good, though. To build up the atmosphere.
Zombies Ate My Neighbors is the best Halloween game. I hope it comes out on the Wii virtual console eventually. It'd be a perfect October release. I remember hearing it was ESRB rated ages ago.
In really great horror games the tension is what always gets me. I can't play games like Silent Hill at night because after about 30 minutes my skin just starts to crawl and I can't will myself to continue forward (yes yes i'm a huge sissy).
I have to say I'm curious how RE5 is going to turn out because RE4 just didn't have that kind of tension for me.
Usually on Halloween I like to play Eternal Darkness and Resident Evil 1 (thank you, Gamecube remake). This year.... I think Dead Space is going to bump Resident Evil.

I am jealous of your setup. I'd never rock headphones with that thing. Then again, if they're optimized correctly headphones can give a good feeling of direction.
