Well the biggest difference is as soon as we stepped through the first door in the game we were getting raped by two bosses and several zombies. Hordes are everywhere now. Not to mention it took is about 6 tries on advanced at the last stage of No Mercy because, let's just say, one tank is not enough. You can NOTICE the difference between advanced and expert.
Just did my first No Mercy full run. PA group on advanced, ridiculously fun. It got about 50x better at the end when, during our final stand waiting for the choppa, the tank ran over and backhanded Roland right the fuck off the building. Just watched his little outline go flying. One of the funniest moments I've had playing a game in a long fucking time.
Myself (Francis) and Louis made it to the choppa, because fuck old people and girls, amirite?

Right as we were going for the helicopter, a boomer came out of no where and vomited on me, then once I could see again, a tank suddenly appeared out of no where almost throwing me off the hospital. Then a hoard of zombies mauled me to death while I was trying to find a way back onto the landing pad.
All while my teammates flew off to safety without me! D=
Hah, just played versus on a pub and got over 1000 damage in one life as a boomer. Vomitting on Zoey and then managing to get in the middle of everyone and the zombie horde so that they couldnt shove me away
The game looks a million times better than the demo. Maybe a zillion. I knew the retail version would look better, but shit, I didn't expect this level of polish.
Some other comments:
Expect to get tons of achievements in your first few hours. Joy.
Playing on advanced for 6 hours got us to the end of the 2nd campaign -- oh yeah "this game is too short" my ass. We even had to tone it down to normal for the end of the No Mercy campaign, after getting our collective asses kicked multiple times on advanced.
Played around with versus as well. Tons of fun.

No Mercy on Advanced: Awesomesauce. The levels are all excellent, and the finale is tons of fun. Took us three tries, and on the third two of us made it out.
Dead Air is cool, some fun parts, but the level design isn't quite up there with NM. The Finale is also less interesting since it lacks the multi level aspect of the NM finale. Still tons of fun overall.
Started Death Toll, but had a crash and decided to call it for the night. Game went from being super fun in the demo to my GotY.
Moment of the night was running into a tank on No Mercy 4, and him knocking two of our guys onto the ledge. He moved to finish them off, but instead took a detour to the first floor.

Its just a viral achievment, play with a dev or someone that has, I believe.
Also valve gets major bonus points for inlcuding color blindness options in the menu, makes the crosshair and lifebars much easier to see.

My group made it to the end of the 2nd campaign but the server tossed us just as we were about to board the boat. We were so fucking pissed.
