The way Rem did it was to monkey with the driver's viewpoint in the truck and put it way behind and above. If actually implemented, it would work more like a typical 3rd person camera would. It would clip against geometry and have a little bit of play so quick changes in position wouldn't be jarring. This would keep it out of places players wouldn't be able to normally go.
Press the talk button for tone. Use team chat and your text is converted to CW and played for everyone else as beeping. If we're feeling generous there can be CW-to-text. If people walk on each other, the mangled beeping is still interpreted and displayed as is.

You could have a toggle for the 3rd person, with the current view being default and a message with the key it's bound to popping up when you enter the drivers seat.
For a totally off the wall idea, what if you hack a sunroof into the truck and stick your head out of it while driving, so you get more of an overhead view without the immersion breakage.

Did you guys ever implement that left hand mode I requested or suggested?
Man come on it would be really easy just find the part where the gun is drawn on the screen and just apply a scale matrix with a -1 value for the x axis and the whole thing gets drawn inverted.

Exactly, this is the Wild West we're talking about here, they didn't take too kindly to southpaws 'round their town back in the day. Discrimination is a key part of the SS experience.
Oh, and it may not be the popular opinion, but I say nay on third person vehicle controls, or at least give us the option to toggle between first and third person views. For whatever reason, I struggle to play any driving section in games without a first person viewpoint.
