They're both quite good, so that's a tough question. New Blood has co-op (for anyone crazy enough to join you) and voice-acting, plus plenty of ideas used in Under The Knife 2, plus the obvoious gameplay tweaks.
Second Opinion has Deftra and Triti, which can be a real headache to deal with, but overall the operations and music were awesome. It's one of the few games where I go through Hard perfecting most of the levels, and is my favourite 'difficult' game.

Oooh, so this is a point-and-click mode much like Ace Attorney? With supernatural elements? *fist pump* My interest is rising again. The music was also very nice, I hope it's included in the main game.
The only offputting thing for me was 'Little Guy' being a Mii, it just looks out of place. I can choose to switch him to my Ross Noble Mii, I'll be satisfied.

No, Little Guy's Mii actually looks like the "real" character it's representing. You can't switch him out.

Man, first NI puts Disgaea 3 only on the PS3, and then Atlus makes a Trauma Center sequel without weird parasite things. What is the world coming to?
Well it's not like this is "Trauma Center 3." This is Trauma Team.. almost like.. a pseudo sequel. In theory they could still release a Trauma Center 3 that continues the plots of the original TC characters more directly, and has more mutant bio-terror and crazy shit like that.
But I think it's a good idea to take the concept and gameplay in a new, fresh direction. We've had four Trauma Center games, all good of course, but something fresh is quite welcome.

More like a spinoff than a sequel. Kinda like all the Law & Order variants.
Yes, I am a boring person who was enjoying his doctoring sim until it got all Sci Fi Channel Original.
From a story perspective, yeah, it was pretty out there.
But from a gameplay perspective, it kicked ass.
Still, they probably could've crafted a good story just treating regular diseases... Would've been really dull though.

I'm curious if there's anything to the 'life-changing experience' mentioned at the end of the trailer (like a plot arc in place of GUILT), or whether that's just referring to the game in general.
Anyway, both trailers are fantastic, colour me excited.
I'm actually on the fence about this. New Blood was a bit disappointing, and the removal of GUILT has me pretty bummed. Still, it is a new Trauma Center game, and the word of a new super-masochistic difficulty setting is pretty exciting.
Does anyone have it yet? (Does anyone still care? Hype for this game is almost nonexistent; I only just noticed it was coming out this week.)

I liked New Blood more than 2, personally. It was a nice change of pace.
2 just felt like more of the same old mega virus drama. I think the series will benefit from not having one.
I probably won't be getting this since I have too many other games going, but I'd like to get it at some point.

So I'm slowly working my way through Second Opinion 'cause I got it off Goozex for a pittance. I'm really quite enjoying it, even when
I thought Egoraptor's take on the game was abit of an exaggeration - now I realize it's rather accurate. (Warning: Spoilers in the link.)
Still - it's a fun game and I'll certainly buy the rest. Trauma Team looks like it's going to be bloody brilliant too.

Some of the scenarios sound awesome (accident scene First Aid/CSI), but some sound decidedly meh (Cosmetic surgery).
Then again, can't bash it too much until I try it.
