Yeah, just to be clear: Dicking around in spawn is A-okay. It's just when you run into Stubborn McJackass who is frantically mutilating himself and telling you to heal him to "get dem ooberz". This happens to me much less frequently now that I've found you fine gents.
So I definitely hurt myself on the way to the battlefield any time a medic happens to be running with me
because I like when the medics kritz me and it happens faster that way
I mean if they're being a jackass about it then yeah but otherwise there's nothing wrong with this approach

He's only talking about in the spawn, when your uber charges at the same rate no matter what.
If you're at a base, trying to build an uber to cap the point or whatever, you damn well better be hurting yourself. Time is of the essence.
I always thought it would be cool to give the medic some sort of weaponized Timothy Leary experiment. Hit someone and make their screen turn into a kaleidoscope drug trip for a few seconds or just stun them like the Sandman but with dilated pupils and a stagger.

The syringe gun should inflict a temporary debuff that randomly immobilizes the victim or renders them unable to fire their weapon. One or the other (they can still fire while immobile, unlike with the sandman). The chance for this to happen depends on how many needles hit the victim over some period of time.

EDIT: You'd be surprised at the bottled rage within the medics. It's part of the job. There's no other class whose job relies so heavily on the intelligence and consideration of the the rest of the team.
I know I am an asshole for it, but this is why I don't play medic.
I need to be responsible for the man killing/ lack there-of.
There's already a brilliant spy counter, it's called perception.
But nope, 80% of players are too god damn blind to the fact that a scout moving at half speed and doing tiny single jumps is, in fact, not a real scout. Even though he's been using that disguise for 20 minutes straight.

Here's the thing that gets me about complaints about spy-checking: unless you're willing to forfeit the match by shifting your entire team to Pyro to sweep every single inch of the map with flames, there is going to be a hole for the Spy to work with. Good Spies don't stalk the enemy when they're alone, they work the back lines of the hottest engagements, where all the various explosions and gunshots will paint over their decloaking sound and where enemies will spend the majority of their time facing forward, paying attention to the men who are actively shooting at them rather than flipping around to catch the Spy that may or may not be behind them. Getting backstabbed in combat is unfortunate (and admittedly still partially avoidable), but it doesn't mean that your team sucks at spy-checking.
If anyone, blame the Medic; he's the only person whose perceptions you can really spare.

I'm mostly concerned about the people who bump into me while i'm cloaked, are stuck against me for more than a second doing the "Move, asshole!" shuffle, and continue on without shooting me.
I just do not understand the thought process that goes through these people's heads.

Not really. Even the Vintage Tyrolean (so awesome) doesn't compensate for the other medic hats :P
And the other classes generally have at least one awesome hat, or a hat that fits the character really well.
But I left out the best part.. pellets that hit enemies put mario kart style ink splatters on their screen for a few second!

The Bio-Regenerative/Degenerative Nano Projector.
Hit allies? DNA encoding makes the nanites heal them and repair damage.
Hit enemies? Unknown DNA, reduce to grey goo.
