You get paid what your grade is. There's no negotiation. For military the methods for increasing your pay are biannual raises (Which are separate from yearly cost of living bumps), moving in to a higher demand billet either because of the nature of the job or its location, or getting in to a more valued job which generally gets you faster promotion and possibly regular bonuses.
For civilians as far as I know their methods are generally vying for promotion. Don't know if they get more money simply for how many years they've been around like the military. Unlike the military they can also get performance based bonuses.

I think that is an IBD(Crohn's and company) thing. I am on remicade which is an immunosuppressant which gets rid of my Crohn's. Be nice if I could skip that.
