He can only get one DAC per Region per phase, whether he gets them during the domination (Creeping Death) or corruption step (cultists) doesn't matter. He did get one for Estalia and one for Bretonnia. No CT in other phases (Summoning/Battle) or other populous regions (Kislev/Empire). So I don't see where I may have missed one.

Also a cultist can die in Estalia.
