We are finally putting in a ticketing system. I'm going to demand a filled out description box or else I ignore their requests, no more subject line emails for me!
I would guess having more pre-defined fields instead of an open text would be preferable. Even if they don't build off each other, having some options to narrow it down might get better results?

The problem is that most users, even today, don't understand the conceptual separation between hardware and software, much less that between software and an "account." Banks have accounts! This isn't a bank!

And then on top of knowing the difference between hardware and software, being able to correctly tell the difference between a hardware and a software fault.
Something like a BSOD? Could be either, hard to tell.
