I watched an NGS playoff game tonight.
There was a Ragnaros.
His team was all in on dive.
Not only did this Ragnaros not go E build and instead chose meatball.
He did not finish his meatball quest until seventeen minutes into the game.
I am so vicariously tilted right now.

#1 Protip: You can hit D even if you are channeling his E.
The basic "combo" in fights is Q->W->E->D.
A lot of this combo is based on timing and tracking how many Q/W stacks you have spreading around, and then hitting D when you get the most value or before the Q/W stacks run out. Don't use E or D unless you actually need them. Even Q needs to be used a bit judiciously (as opposed to other healers who just spam their heal button on cooldown). A 10-second cooldown can feel like an eternity if you don't have a way to heal your team when a fight breaks out, and his heals take even longer when you consider that if he wants to heal more than one target, he has to wait for the spread or he has to wait another 10 seconds (until CDR talents kick in).
Stukov's E is particularly brutal if applied at the right time and the right place. For example, if the enemy team dives your team, putting W+E+D under them stops the dive in its tracks. Hits ults are similar in that, if applied on the right target at the right time, they can effectively stop the enemy team from doing what they want, such as diving or casting their ults.

WE WON ROUND ONE OF THE PLAYOFFS FUCK YEAH

It's rather straightforward and very effective. You get a big ol' Silence puddle that hits harder and lasts a bit after being cancelled. I feel this build is definitely the easiest one to play. Take whatever you need at 4. Vigorous is good if you just straight up need the extra burst healing, One Good Spread is good if you want more uptime on your Q, Biotic Armor is great vs AA Heroes. Can't go wrong with your 4s, but I do lean towards One Good Spread as a go-to. Both Ults are good, it's just a matter of do you need peel vs *one* particular Hero (Massive Shove) or the entire enemy team (Flailing Swipe). You can also try taking It Hungers at 13 if you want, that one can be pretty effective. Virulent Reaction is also very strong but also VERY hard to pull off (need to line up 3 rather long cooldowns).
This one you definitely take Vigorous because of the synergy you get from Targeted Excision. A little trickier to play, but not that hard once you've got a solid grasp on The Stuk. I tend to grab It Hungers with this build over anything else at 13 simply because there isn't really much benefit for Lingering Spines in this build. Mostly I take this build when I need to have a ranged AA. Spine Launcher is great, gives a bit of minor CC too, but I feel like if I take it I shouldn't really be building into E build without Low Blow.
Stukov's autoattack is really REALLY good though and it hits like a fucking truck even at level 1. But, yknow, it is melee. And sometimes that's dangerous! Hence Spine Launcher. But otherwise, keeping that hefty melee AA is really beneficial because it is a surprising amount of damage.
OH YEAH and one last note, if you take Flailing Swipe, Controlled Chaos at 20 can be a good pick. I don't really like the Massive Shove upgrade though. Bio-Explosion Switch is...... nehhhh. So really that leaves Top Off, which is fine, because that talent is fuckin' bonkers good.

Low Blow being the talent that makes E worth building is an overstatement, I think. 7 and any of the 13s are plenty strong without it.
Excision + Vigorous is good but less fun, since you have to wait for your other abilities to come off CD. Taking the CDR at 1 and 4 means you can D three+ times in a fight, which is hilarious.
There aren't any Stukov builds, his role in the team is the same no matter what talents you choose. You keep your healing pathogen on your team and look for good silences to land on the enemy team. Individual talents change based on what the enemy team is doing but that's a matter of asking yourself questions at each tier.
1) Is it going to be ridiculously easy to land W on the other team? Take the W quest talent otherwise take Low Blow. If you're getting dived on where Reactive Ballistospores is getting use, you need to work on your positioning, not take different talents.
4) Take 1 good spread. If the enemy team is entirely physical damage, you can take the biotic armor but that doesn't happen that often.
7) Take growing infestation. Your main role in fights is not messing around with your Q and W, it's landing a good silence that disables their team. Take the talent that makes your best ability better.
10) Is the map one where you can push 1 person forever? Take shove and try to make fights 4v5 by pushing their tank out. Otherwise take swipe but neither ult is that impactful.
13) Are you confident that you can hit your W on a target that matters? Take the root talent, this is stukov's most impactful play and can easily secure a kill. If it's unrealistic to land your W on target (like facing a johana + 4 backline), take the increased range for your silence.
16) Does the other team have stuns? Take Super Strain otherwise take Pox Populi. Easy choice.
20) None of the 20 talents matter too much. Top off is reasonable, never take Bio-Explosion Switch.

ehhh, that's not true, cmon
i will agree that his talent choices can feel a bit lackluster at times, particularly his.... 1s, 13s, 16s, and 20s? like just in a pure "feel" kind of way, not a numerical/data way. a lot of his talents don't feel good to take, so it ends up that even though he might have 4 talents on a tier, he really only has 1 or 2. like, for example, i never take anything but Pox Populi at 16 and Top Off at 20. Superstrain and Universal Carrier may be performing on par with Pox Populi, but they don't feel good to take at all.
but he absolutely has builds. there is numerical data to back that up! i'd say he has 3 distinct builds at the moment, E build (Low Blow/Growing Infestation), W build (Fetid Touch/Long Pitch), Trait build (Vigorous Reuptake/Targeted Excision), and there's even a bit of mix'n'match you can do for the appropriate situation (like taking Spine Launcher as your level 1 over something else, etc)
Also, I find it best with Stukov to self-cast your heal and walk to people to spread it. Can't count on teammates to spread it back to you or others. I have a special button binded on my mouse just for self-casting heal (works great with Arthas too!)

One is Targeted Excision, for maximum throughput. Great against poke. Best paired with Fetid Touch at 1. Requires incredible confidence in aim with W.
Another is Virulent Reaction, for maximum lockdown. Not nearly as much healing, but enough for most fights. Almost requires Growing Infestation at 7.
Yet another is Poppin' Pustules, for maximum hilarious teamfight damage. Not a build I'd try in a serious game, except maybe against Samuro. Best paired with Fetid Touch at 1 and The Long Pitch at 7.
And then both ults are viable in any build, but usually I take Swipe, as it's a more thorough and safe disengage. Shove is only good if you know your team can protect you during the duration, otherwise you've just killed yourself.

I'm not being down on Stukov at all, he's a very impactful healer and landing a combo post-13 requires good mechanics and good positioning. I've personally seen you pull out some great stuff with him. It's just that his talents don't change his core gameplay and thus you pick more reactively then deciding that you're going for a certain build. It's like Raynor. No matter what talents you pick, you're still going to be right clicking on enemy heroes and filling them full of daylight.

I'm not being down on Stukov at all, he's a very impactful healer and landing a combo post-13 requires good mechanics and good positioning. I've personally seen you pull out some great stuff with him. It's just that his talents don't change his core gameplay and thus you pick more reactively then deciding that you're going for a certain build. It's like Raynor. No matter what talents you pick, you're still going to be right clicking on enemy heroes and filling them full of daylight.
I think we're just going to have to agree to disagree here. While it's not a build you can use every game, and I even probably wouldn't touch it in NGS (team league and below are totally fair game though), I do think Excision does fundamentally change the way you play him. Positioning, target prioritization, and cooldown management are all different. Hell, it even introduces mana management to a character that is virtually impossible to run out on otherwise. That's why Dunk played it when E build was already the established meta, and I adopted it from him. Pushing buttons is fun, basically, and I want to do it more :P

Butcher reaches a point where his stacks lock in and he can start playing more aggressively. Alarak gets sent back to square 1 if he ever dies. Well, except for Alarak's optional lightning quest stacks.

Yeah, E Build Alarak literally doesn't care about his own damn Trait because the Lightning stacks start to outpace Sadism really hard.
I think Alarak's Trait could use another once over, honestly... Like even just giving it the Wizened Duelist/Redemption/etc treatment (lose some stacks but not all). Well. Alarak in general could probably use another once over.
Also Alarak is fun but BOY is he hard to fuckin' play, man. Half the time it feels like I just can't do shit with him, I dunno. I'll try giving him another try but man.

I've got two builds for you @21stCentury!

Butcher is a binary hero. He is either deleting your team or he's dead. He's either above 200 meat stacks and a monster or he's below it and he's manageable.
Butcher is a hero that preys on the QM chaos to deliver him zero counters.

Alarak is another one of those feast or famine characters that you want to kill every now and then to keep him from building up too much momentum. His abilities start to hit really damn hard once he builds up enough stacks.
Butcher reaches a point where his stacks lock in and he can start playing more aggressively. Alarak gets sent back to square 1 if he ever dies. Well, except for Alarak's optional lightning quest stacks.

Yeah, E Build Alarak literally doesn't care about his own damn Trait because the Lightning stacks start to outpace Sadism really hard.
I think Alarak's Trait could use another once over, honestly... Like even just giving it the Wizened Duelist/Redemption/etc treatment (lose some stacks but not all). Well. Alarak in general could probably use another once over.
Also Alarak is fun but BOY is he hard to fuckin' play, man. Half the time it feels like I just can't do shit with him, I dunno. I'll try giving him another try but man.
