Yup. Sometimes on a miss, sometimes on a hit. A critical hit will cancel reactions.
If you get multiple hits you have to carefully choose which to resolve first as a reaction can cancel unresolved hits if the monster moves or the attacker dies.
On some hit location cards, yes. Some will react no matter what. "Failure" ones only react if the hit fails to wound. A critical hit will cancel any reaction.
Without reaction attacks, players in the blind spot would be invincible.
I just ask no spoilers or allusions to events or monsters bosses to come. Even I'm flying blind on that.

It's mostly like to so you're not a threat the next monster turn, to prevent it from just focusing one person down.

Pretty sure you can dodge reaction attacks if they are a normal attack with a hit roll etc. No dodging that grab though.
Don’t forget the critical leg injury!
