The way it works is that she's definitely facing left when she's left and right when she's right, which is why most people don't believe she could possibly be rotating the other way. The trick is that given the clever angling of her body, it's possible for her to be rotating either TOWARDS or AWAY from the viewer between the left and right hand positions. If you open it up with a gif editor you can see what's happening.

Start at frame one
Look at the positioning of her shoulders and head
do you see how she could either be looking towards the screen, or away from it?
progress the gif a few steps, and you will see that depending which path your mind chooses, she'll appear to continue down; she either rotates counter clockwise or "upwards" to reach the straight right position, or clockwise and "downwards"
argh that fucking spinning girl distracts me for a good 20 minutes every time I see it
jesus christ it kind of makes you distrust everything you've ever seen in your life
Also this is a similar thing to the grayscale waterfall video, stare at the cross
The blinking dot should start blinking green and all the purple dots disappear
(edit) Also here is possibly a good trick to making the spinning girl change direction: when moving 'clockwise' her foot will move downward, as though the camera is placed vertically above her ankles. When "counterclockwise" it's more like the camera is vertically below the ankles so the foot appears to swing up as it comes towards you.
Also I just realized that when moving 'clockwise' her right leg is out, but when 'counterclockwise' her left leg is out jesus christ this is complicated

If you're trying to make it work, not wondering HOW it works, just look away for a few seconds and then back. Once your mind establishes a "path" that she's taking, it's very hard to undo it while still looking at the image.
I couldn't for the life of me see it going clockwise like everyone's saying, till I opened it in ImageReady and slowed the speed down to .5 sec a frame and just looked at from calf to thigh. Now I can finally see the illusion. At full speed my mind sees nothing but counterclockwise.

Now I just get sorta sad when one of the directions goes away and i can see the other one
EDIT: for shits and giggles, see if you can get it to rapidly change directions. Its kinda fun, and i can almost change directions at will now

then go through it from the first frame imagining counter clockwise
