1 ) Bad placement in the first place. Who's bright idea was it to put spawns in open areas? See: Burial Mounds.
2 ) There's no particular rhyme or reason on Objectives on what consitutes the enemy being in your base enough to make you spawn elsewhere. Take Headlong for example. Sometimes it only takes one guy to make everyone spawn at the Statue, but most of the time ALL of us will be in their damn base and they're still spawning at the back! WTF!? Coagulation and Waterworks do this sometimes too. And Containment just seems to spawn you at the trench sometimes just for the hell of it.
3 ) The game apparently had no spawn logic in the sense that if you die in that spot immediately, it will more than likely immediately put you back there. Sanctuary is really bad for this - it'll keep spawning you in the open part of the base as a sniper just picks you off from the middle. Also, there's the weird spawns where you'll spawn right behind an enemy, or an enemy spawns in front of you.
4 ) Spawning people next to each other in Team Slayer. On some maps it never seems to keep you together, but on maps (especially Ascension and that fucking banshee platform), it always makes you spawn together.

There is some arbitrary line near the bases where if everyone on that team is not within the line and an enemy is, they will start spawning outside. It's different for every base so it makes for lopsided lock downs and force spawns. The problem with the slayer matches is that when you get shot for far away, the game thinks, "hey, there is no one around here, I'll spawn here." Which just puts you back in the same area to get sniped again.
I don't think making completely safe spawn zones will solve the problem. Unless each zone has more than 4 exits, the enemy will just camp the few exits/choke points (see R6:Vegas for example). The best way would probably be semi covered zones with a respawn grace period where you are invincible (some maps in Lost Planet are like this). Unfortunately this will make objective games really hard if you have invincible players spawning right as you are trying to steal the flag or plant the bomb.

This one doesn't make any sense. Air traffic controller? An achievement for shooting a banshee with a rocket? This does seem fishy.

Oh man, it's horribly tough. At least there is no achievement for Nightmare, because I recall the devs saying DOOM wasn't even meant to be beaten on that difficulty (not that it's impossible, but they didn't bother to make sure it was doable).

There is ONE achievement for Nightmare on Doom. Beat a level.
I played through E1M1 on nightmare and got like like 5 achievements in one run. But that one run was a bitch.
Each character did, but they intersected and stuff, and tehre was a chronological order of sorts, chapters and things. I basically want something I can link to a friend and say, "Listen to these in order."

Ilovebees has them in order. I gave my friend the first half and told him to listen to them by week and by subject.
Also, as someone just found out, right now my connection sucks (constant redlining) So I'll be back on once everyelse in my house gets off the net and/or goes to bed.
That doesn't sound like a complete or accurate achievements list. There's supposed to be an achievement for clearing the game on Legendary - it's called "Legend" - and it's there, but where's the achievement you're supposed to get in multiplayer by killing someone who has the Legend achievement?
