It's not a joystick it's a "Command Unit"

So with the world weary years of my thirties now firmly entrenched upon my soul, I find that using the keyboard for extended FPSathons ala Oblivion and TF2 really does a number with my keyboard hand. Simply the position that my hand has to be in causes pain after too long. (The keyboard has to sit in behind the mouse instead of beside it due to the triple wrap around monitors. [dedicated gamer for life yo])

I went out and bought a 'Cyborg Command Unit' from Saitek, normally I avoid Saitek, preferring the industrial grade ompfh of CH Products but since they don't have a comparable unit, and this was a mid price option for something I had no experience with I picked it up.

After several months I don't have any more pain, but my game has suffered. The keys are simply too sticky and unresponsive. So it is time to look at other options.

What should I be looking at in this field?

    You loose buttons, but maybe try using a controller for movement?

    Not a bad idea. It'll only work if you can program in keys to joystick axis. Unfortunatly it won't work, as Win 64 locks out any thing I do to my controllers, so I can't reprogram them.

    If he's looking to avoid pain the Wolfking is probably the worst choice. I bought their full size keyboard with that attached on the left side, and absolutely hated it. It's a great idea in concept but clearly wasn't designed for hands of the human variety. It makes some keys a lot more accesible, yet others that used to be easy to press now require hand contorting maneuvers.

    The keyboard now resides in my closet.

    Get a Nostromo Belkin, for sure. I use it for CSS and UT, formerly WoW too, never had a problem. I try to set it up for every game I play. My friend uses it for UT, competitively as well, and he fucking loves it.

    A note about the new version (n52te), if you really don't like that little stick on the D-Pad, you can just pull it off.

    Never used one myself, but this funky gadget might be what you're looking for:

    Basically you can stick the buttons in any configuration on the base to suit your hands.
