I recently dug up an old DnD campaign I started writing a year or two back.
Because I'm interested in involving my players in the campaign, I put together a packet for character Metadata,
all sorts of information about a character that could be useful to the DM in any stage of the campaign implementation.
As it took me several hours to plop the PDF together, and it's something that may be useful to others, I thought I'd link it here. All of the text fields are fully enterable via the computer.
The Character Metadata Sheet is a packet for organizing character information for tabletop gaming meant to accompany the Character Sheet.
I created it to help myself tie character backstories into my campaign, as well as act as a resource for the players.
It is a good aid in character creation, and developing a roleplaying identity.
All of the fields are text-enterable.

Very nice - lot of good stuff there. I belong to a roleplaying club which uses a wiki for character bios and suchlike, so that a GM can add personal touches to the campaigns. Nothing makes you desperately improvise like meeting the woman you abandoned at the altar many years previously.
Its cool and all, but I don't see that many people using "Background" sheet like this. If we would like to go that far into building our characters, I believe we would just write it in a Background and Personality spots on CB.
The main idea for me, was to hand this out with a lore packet I created (because I figure, if these people have supposedly lived in this world, they should know a little about it), and let them plan their characters long before the start of the actual campaign. I'm hoping to get all my sheets back so that I can tie them in (improvisationally or concretely) to the actual story.
It's not really something I expect the characters to have with them each session like a character sheet, unless I wanted to be super hardcore and have them only allowed to share character story through in-game conversation.
I'm sure it wouldn't be of much use to someone who's midway through a campaign.
