Would you guys and gals agree this is the best primer for updating New Vegas for a totally new playthrough? It's been a few years since I've played Vegas, and have never really modded it before. I want to keep things as simple as possible, I'm looking for...
New Vegas Redesigned 3 has come up a few times as recommended, but it has an AO warning. For those that have used it, would you say it's about the level of the Witcher 2?

AO warnings usually mean straight-up nudity, so it's up to you :P
I think the easiest and most drastic visual changes you can make to the game is an ENB injector and skin texture replacements (not necessarily additions, but replacements). The base lighting engine makes yellow tints horrendously saturated, and thus vanilla Asian characters (or other pale-skinned characters for that matter, really) look like they all have jaundice. A nicely-configured ENB preset can turn even the most basic of New Vegas installations into a much more atmospheric-looking sandbox, and depending on the lighting options you take the difference in visual quality can be night and day (heh).
I cycle between many of them depending on my preference at the time when I pick up Vegas again, but my pet favourite presets are ENB of the Apocalypse and Desert ENB if I'm feeling in a filmic mood.
I also enjoy the rich-colored, more saturated ENBs every now and then but unlike other games with their ENBs I actually feel having a slightly more muted visual palette fits Fallout a little better.
What about redone faces? I'm not expecting everyone to be super models, but I agree that the Fallout 3D engine is not the best at them. Is there a balance between new and old?

There are certainly face replacers out there (in the height of my modding phase of gaming I used to spend literal hours in FaceGen for player characters, which is a program that translates head morphs directly to Fallout games without very much 3D modelling experience at all).
Unfortunately, the vast majority of face replacement mods are, you guessed it, supermodel fantasies. :P Thus, I don't actually use them all that much, although there's a decent variety of face texture replacements, some of which are even lore-friendly in a sense.
What looks like your safest (and probably one of the only) bets for great face replacements is this mod, which also seems to be the most highly-requested/downloaded, as well as the most comprehensive. A lot of the screencaps look pretty photogenic, but on the upside compared to the scores of mods below it, they don't appear to be in full Hollywood mode.
The funny thing is that the supermodel faces would work fine for androids and be believable. Maybe have a few with their skin falling off and revealing robotics underneath.
But, back to the honest wishing, whatever comes out of E3, I would prefer an engine that makes combat more fun and AI more dangerous beyond being bullet sponges while little else in the engine changes (at the beginning).
the number of people who make mods to turn fallout/elder scrolls into creepy sex simulators is too damn high.

Not really a fan.
And the thing that tends to be a pain is that even a lot of the cool lore-friendly stuff (even the outfit I got in the picture above) tends to require a body model replacer of some kind. A lot of them require Type 3 replacement, and often the listed replacement is a bit... excessive :P
The first one there is what I installed. Maybe I did it wrong.

Iirc translucency issues (especially with grass and first person view objects) can be fixed with some .ini rejiggering, but its been ages since I've done it so I can't say for sure

Watching a blind lets play of Fallout 1.
Its quite surprising actually. Not only was he playing with low int for the longest time but he actually managed to get the water chip and make it all the way to the brotherhood and get the implant to become smarter without a whole lot of direction. He did pretty well.
Of course now comes the fun part where he just recruited Ian, got super excited about it while I chuckled as his unarmed heavy vault dweller and Dogmeat started to get shot in the back repeatedly.

There's an absolutely fantabulous, extensive and lore-friendly (in a sense) People's Radio of China station mod, but I only have that loaded because my last playthrough was roleplaying a generational ex-Commie :P
Also because apparently installing things in the wrong order will shatter the space time continuum, is there a general rule of what to install first? I'm assuming structual stuff first, then textures and lighting, then additional stuff like weapons and NPCs.

Before you install anything, start a game and run through the intro till you leave the house. Choose a basic character you like. This is your base save that you will come back to. Make new save games from here on out.
Skim gopher's series for titles of what to pop in next, it saves a lot of headaches and the only tutorials you should really, REALLY, I mean seriously listen to what I'm saying and watch them, are the ones about mod managers and bashed patches.
Your order is sound because a lot of that stuff is loaded in the base game while others are only spawned when you get in range or meet certain conditions.
Adding NPCs and enemies are best added last because you run into the situation of spawns and cells not resetting.

Logic is definitely sound. Base games and expansions always, always at the top. Simple replacements of things less likely to break the game first, then larger, more game-changing things like conversions and stuff towards the back.
Most modders worth their salt should have suggestions as to your loading order too.
Oh goodie, ready to start modding in some goggles and scuba gear, and it turns out I do not have python installed on this desktop since the last upgrade.
Then there's PyFFFIT or some shit, and then teh Blender expansion.....I'm almost ready to quit, then I remember I have a vision, and that vision says people in wetsuits and speedos trying to kill me on an oil tanker in a game within a game with communist sympathies coded throughout.

I don't personally mind having an attractive player character (I wouldn't have wasted away hours in FaceGen otherwise) but I like to keep it somewhat within the Fallout aesthetic. (Don't ask about the weird hand seams).
And don't get me started on the Poses and Animations section. Just... avoid that hive of scum and villainy.
