Having decided I'm either going to PAX east or prime next year, Ive also decided to (FINALLY) buy a DS. So what games do y'all recommend? Bearing in mind I'm in the UK

I like the DSi. I'd say it's worth it for future-proofing yourself for DSi-only games, being able to take advantage of the DSi shop and such. Flipnote Studio is really awesome to play with, and I have already used the built in browser a ton - I'll be playing FF4 DS, save and quit, flip over to Gamefaqs to look something up, and load it up again.
Of course there will shortly be four people in to tell you not to buy it. It's up to you!
Although its not multiplayer and you won't be playing it at PAX, every DS owner should own Mystery Dungeon: Shiren the Wanderer. An awesome handheld Roguelike that has given me the most playtime of any DS game I own.

Hmm. I enjoy it somewhat, but it doesn't hold up to my roguelike standards of something closer to Nethack, with lots more equipment and options at any given time. And the dying and restarting does get very old, you feel like you can never use any of your stored equipment since that risks a freak accident that would destroy it.

Hear fucking hear on Professor Layton. Buy a used copy, beat it and return it, should run you a net loss of $10 for about 20 hours of good brain-wracking puzzliness. I also really enjoyed Hotel Dusk: Room 213, but again, this game has 0 replayability.
Zelda: Phantom Hourglass is the best DS game I've ever touched, though. Period.
The World Ends with You is an amazing game, easily my favorite RPG on the DS. And, if you take it to PAX, you will level the everloving shit out of your pins (attacks)!
If you like strategy games, the Advance Wars series.
If you like RPGs, Chrono Trigger and possibly Golden Sun if you can find them (they're GBA games though, so you'd need a lite or classic DS, not a DSi)
If you like both, Fire Emblem.

Star Fox Command. Fun Star Fox title, plus really really fun with a group of people.
Pheonix Wright and Professor Layton games are a MUST. I love both of these. Oh yeah zelda too. Brilliant DS on the go gaming. If you like shorter puzzles then try picross. That can be addictive

It's supposed to be really good, it just had the misfortune of being launched along with Scribblenauts and subsequently didn't get as much buzz as it should have.

After the first couple of realistic missions it turns into a game about special strains of viruses and fighting terrorists and all sorts of stuff. It's a FANTASTIC action/puzzle game and I would highly recommend it, but don't expect it to be a simulation.

Bowser's Inside Story is awesome and the best of the Mario & Luigi trilogy, imo.
It's hilarious, and actually did awesome in sales.
Pretty much any Harvest Moon except Islands of Happiness. In fact, if you have a DS and DON'T have a Harvest Moon game, there is a special group out there just to kill you.

Since you're headed to PAX, you ought to have some multiplayer games. How about Phantasy Star Zero, an action-RPG? Pokémon, a strategic dueling RPG, will definitely have a presence. These are also playable online.
There are so many great DS games, but I'll try to avoid the repeats:
Rhythm Heaven is a great mini game collection. You really can't play anywhere that's too loud because you need to hear the music to be able to play it, but it's some great fun.
Ninjatown is a very solid tower defense game and is frequently hilarious.
Sonic Chronicles is a decent RPG which is actually very reminiscent of the Mario & Luigi games (but it's not as good as Bowser's Inside Story. Get that one first!)

You're thinking of Trauma Center, but a warning: it is NOT a game that makes you feel like a real surgeon. After the first couple of realistic missions it turns into a game about special strains of viruses and fighting terrorists and all sorts of stuff. It's a FANTASTIC action/puzzle game and I would highly recommend it, but don't expect it to be a simulation.
It's still a very very fun very challenging game, but yeah it's definitely not "realistic" at all. Super fun though.
Also, speaking of which, I still crack up when I watch this even though I've seen 100,000 times:
- Professor Layton And The Curious Village Both professor laytons are incredible, they each have a lot of filler tedious puzzles, but it is definitely made up by many amazing puzzles and design of the game/atmosphere. As mentioned before, its good to rent, not much replay value.
- Professor Layton And The Diabolical Box
- Scribblenauts - Alot of fun, but I stopped playing it after about 1.5 weeks, I'm not sure whether it was because I got bored of it, or because I got busy. Even still, in those first 1.5 weeks, it was like "the best game ever" in my mind.
- Chrono Trigger - The best RPG, but this time you can play it anywhere! Very well made, some irks like they change names of items for some reason, like tonic to potion. Oh well!
- The World Ends With You
- CrossworDS - This game is nice, I spent a surprising amount of time on it. The user friendlyness is very nice, kinda like picross ds.

- Zelda: Phantom Hourglass
Here are games I feel are overhyped.

- Final Fantasy 3
- Final Fantasy Tactics Advance 2
Bullshit. Castlevania is the best thing about the DS. I could play those quality SoTN spin offs until I die, in my mind each one is a beautiful little gem in the crown of the original. I'm in the middle of A2, right now and I think it's fantastic. Scribblenauts is fun for like 10 minutes, TWEWY while fun to look at and a well delivered RPG is probably one of the most annoying games I've ever played. All those items, and that music. Graugh! I agree, however, with your assessment of Chrono Trigger.

Truth, you really can't go wrong with Harvest Moon. It's just the most perfectly formulated game for the DS.

I mention skipping Islands of Happiness because you're forced to use the stylus controls in it and they're not very cleanly done. Sunshine Islands, the sequal, is a "fixed" version of IoH with a bunch of additional stuff.
My favourite is still Harvest Moon DS. It's probably the densest Harvest Moon game out there although I will always have a special spot in my heart for More Friends of Mineral Town. What other game lets you marry a monster that wants to suck out your organs through your anus?
