There was a computer game I used to play all the time when I was younger. I can't remember if it was on our old DOS computer or the one that ran on Windows 98, but it's name has evaded me for years. It was a top-down adventure type game played on a grid of tiles. Each map was a different shape and as you moved your character around the tiles would flip over and reveal what was hidden beneath them, however stepping on the same tile 3 times would cause you to fall through and die. The point of the game was to uncover the key and the door and escape, all while being chased by a monster and not falling through tiles. I don't think you had any kind of weaponry to fight the monster with, it was mostly just a mad dash for the door. The real draw point of the game to me was the death screen which was a big red curtain coming down with the words "It's curtains for you" imposed over it.
If anyone could even give me so much as a clue to what the name of this game might be, I would be forever grateful.

That sounds almost like Moraff's Entrap, but I don't remember a death curtain like that.
edit: Upon googling, I think Entrap may be a bit older than what you're talking, I think you were only able to step on a tile once in that game.
