My Dad just got a new Vizio LCD TV, now he just hooked it regular COAX cable into the back of the TV and was able to get about 6 HD channels (Local Channels) just with a direct connection I'm assuming with having a QAM tuner in his tv.
My question is: Is there a quality difference between direct coax connection versus a cable box with a HDMI connection to the TV? He doesn't want a bigger package from comcast he has a $8 basic package and is wondering if there is any picture quality difference.

Probably not, if the TV has already got the HD receiver built in.
The problem would probably be that in some Cable provider systems extended content (eg additional channel packages) are registered to the Cable box's Hardware ID/Serial Number, so they wouldn't be available until the Cable was routed through the Cable box and then to the TV.

No, not for the basic package. He's fine with the analog coaxial connection, and yes, your assumption is correct that the HD channels he's receiving are over-the-air broadcasts.
