Urgent: Class Survey for someone non-USA!

I have to get someone who does not live in the US to fill out a 48 question survey (answering 1-7, it's a 15 minute commitment) and the person I had fell through. If you can help please give me an e-mail where I can send you the .pdf. This is for a cross-cultural communication class. Many thanks.

In addition to the survey, if you would give me a quick paragraph on your age,gender, race, nationality, and describe your cultural environment (rural, urban etc.) If you can help *please* PM me or reply asap. My friend has left me in a bind.
