Am I the only one that has certain music and games linked together in my mind?
Back in 2001 when I was playing alot of Phantasy Star Online...er...online on my Dreamcast I would listen to System of a Down's album, Toxicity. The bassline/beat of alot of the songs would actually often actually match the walking cycle of my character. I played the music and game in tandem so much that now whenever Toxicity comes up on my iPod I'm instantly reminded of PSO's caves & mines areas.
Also, in 2005, I happened to buy both "Legacy of Goku" and The Chemical Brothers' "Come With Us" on the same day, and as with Toxicity, I listened to the album while playing that godawful game. So, unfortunately, now whenever I hear Come With Us I'm reminded of Legacy of Goku...
Well I used to put on Surrender by the Chemical Brothers for playing both Final Fantasy VII and doing endurance races on Gran Turismo 2. I'm pretty sure listening to that album would bring back some memories.
Around the time Pokemon Red and Blue came out I also started listening to the radio. As a result, every time I hear this song I think of riding the bike in Pokemon.
I never listen to music while playing to games for many reasons, so I associate game music with games and I will never forget the Battlefield theme. The ultra long login for BF2 made sure of this.
I always listen to music while playing games. No matter how good a game's soundtrack might be, if I have the option to turn it off and listen to my own stuff I will. There are some exceptions. I enjoyed the soundtracks for Mass Effect 2 and Lost Odyssey quite a bit.
Any Korn from the first four albums will probably remind me of Everquest since I was obsessed with them at the time (around 99-2000). Other things that remind me of old EQ: Taproot, System of a Down, Tool. Though I did go back to play it when they started the progression servers back in 06, so at the time I was listening to the then new album by Blind Guardian, A Twist in the Myth. So stuff from that will also remind me of EQ. When playing on this server I specifically added some Korn back into my playlist to heighten the nostalgia. It worked eerily well.
I played WoW for a while and listened to a lot of music, but there are a few things that remind me of it specifically, such as anything by Ayreon, Shaaman's album Reason, Shadow Gallery, Avantasia's album The Scarecrow.
Certain Symphony X and Stratovarius songs will remind me of Desert Combat.
I remember when I got Tomb Raider I had bought Mighty Mighty Bosstone's Let's Face It and had it on in the background while I played. Now whenever I hear that damn CD I get memories of Lara's first adventure.
Rogue Squadron on n64 gets associated with South Park's Chef Aid album.
GTA III with Welcome to the Jungle, GTA Vice City with Paradise City, and GTA IV with Q Lazarus's goodbye horses.
While playing Mass Effect 2 I always watch The Unit tv show at the same time
I haven't noticed it happen as much recently with newer games, but I do know that anytime I hear a song off Linkin Park's Hybrid Theory album, I think of Counterstrike 1.6, and anytime I hear a song from Creed's Human Clay album, I am reminded of Starcraft 1.

I have Alice Cooper's Classicks forever linked to Illusion of Gaia. I still can't hear House of Fire or Stolen Prayer without picturing Will or Freedan slashing through a dugeon.
I tend not to listen to music while gaming, unless I can't stand the game's sounds (La Pucelle Tactics, I'm looking at you) or there just isn't much music to speak of in a long game (A lot of old PC rpgs where there might be a bit of music, then nothing for a while).
So, the ones I can think of off the top of my head.
A few albums by the B'z, specifically, Best Treasure and Best Pleasure always make me think of Might and Magic 6 and 7. Rock and Roll Part 3 and Spending Time on the Borderline by Ozma, always make me think of La Pucelle Tactics. Which is kind of a shame. It was a fun game, but I enjoy those albums more.
Actually, system of a down, and staind always drop me into the mindset I was in when I was playing Graal. It's a weird feeling, and I'm kind of ashamed of it.
While I'm not sure if these count (because they were actually on the game's soundtracks) but Street Sk8er for the PS1 is basically what got me into Ska music (in fact I think that game spent more time in my cd player then in my PS), and I had never even heard of The Crystal Method before playing NO2: Nitrous Oxide.

I never listen to anything except the game's music when playing a game and when I watch a friend play a game, it annoys the everloving shit out of me when they load up some dance music while playing Oblivion or something. I don't know why, but it does.
GTA: Vice City. Anytime I hear just about any song from that soundtrack, I think of that game. Incidentally, I also associate a specific kind of dove bar soap with it.
Counter-Strike: Source evokes the taste of rye. Good times.

Oh, in the GTA:VC vein, every time I hear Autograph's Turn Up the Radio, I think of Alpha Protocol's boss fight.
Recently as well I've started picturing my characters playing the song whenever I hear something from Rock Band play on the radio.

I Saw Her Standing There (The Beatles) always reminds me of Goonies II (NES).
...And I have no idea why. I mean, I know why - I listened to a lot of The Beatles when I was a youngster (I was 8 when I played Goonies 2 on the NES). I just find it odd that of all the songs and all the games... these two always remind me of each other.
Oh! And another oddity - Yello's entire Flag album was pretty much my custom soundtrack to Another World, Street Fighter II, and Flashback.
I don't have it with music. I don't listen to music while I play games. I find it too distracting. But for some reason I never found having TV shows on in the background distracting.
Before I moved out of home I had my TV set up right next to my PC monitor. I'd stick in an entire series like Scrubs, Great Teacher Onizuka or Band of Brothers and just have it play through. As a result, whenever I watch Scrubs I'm always reminded of Oblivion, which is what I was playing at the time. Band of Brothers reminds me of The Witcher, and Stargate SG-1 reminds me of Vampire: The Masquerade - Bloodlines.
Whenever I think of Twisted Metal I hear Rob Zombie in my head. So many nights spent playing that with my friend years and years ago.
Halo: System of a Down and Avenged Sevenfold, a friend of mine had a mix cd he'd toss in whenever we threw down in system link. (Which was all the time)
I played like 800 hours of wave race 64 with offspring looping. I don't really know why. My friends at the time didn't like wave race (which I LOVED) and didn't like offspring (which I still like, taste be damned). I almost never hear the songs I listened to (it wasn't a real popular album I had, but I can't remember the name of it), so it doesn't trigger that way; but whenever I'm digging through my gaming stuff and find wave race or the box, or when the newer one came out and I would see a commercial on TV I would always start singing mota boy to myself.
I don't like to over-write game music from like final fantasy games, but music is so important to everything, to me. The final fantasy music is 50-60% of my enjoyment of them. I really like older movies more than newer ones because they almost always had a score; not just rockin' music when the action kicked up. Outdoor activities, or games like racing games or whatnot always seem better to me with some music playing; and if I can set it up consistently (like offspring with wave race) even better; strengthens the memories of both. I don't try to analyze (like x music really fits x game!) even; whatever music tied to whatever event seems to have so much better memories to me.
