In Vermont
Every Vermonter could sign up for state-financed health insurance under a bill passed by the House on Thursday that would put the state on a path to a single-payer health care system by the middle of this decade.
"This bill takes our state one step closer to a system that ensures that all Vermonters have access to the care they deserve and contains costs," House Speaker Shap Smith said shortly after the House passed the bill 92-49.
The measure now goes to the Senate, where it is expected to pass, but with some possible changes.
Our friendly northern neighbors know all about single-payer health care, but for the uninitiated, here's a
primer (from an admittedly biased source):
Single-payer is a term used to describe a type of financing system. It refers to one entity acting as administrator, or “payer.” In the case of health care, a single-payer system would be setup such that one entity—a government run organization—would collect all health care fees, and pay out all health care costs.

Under a single-payer system, universal health care for an entire population is financed from a pool to which many parties--employees, employers, the state--have contributed. Vermont's governor, Peter Shumlin, campaigned for office on implementing single-payer health care. It has to jump through some hurdles because of existing US law (including Obamacare), but at this point none of that seems insurmountable.
Vermont activists have been fighting for single-payer for a long time. There are also active campaigns for single-payer health care in
California,
Oregon,
Pennsylvania, and
New York, and there's at least some presence of the movement in every state in the USA.
I think it's pretty rad.

Our single payer is called Medicare which is clearly listed as a section of your income tax. If you use it more than a specific amount you pay a little extra, and if you earn less than a specific amount you pay a little less.
Also individuals or familys are welcome to purchase additional health insurance which can be used to recieve care in more expensive "private" hospitals and can also contribute to things which medicare doesn't cover like dental, optical, physiotherapy etc etc or things which medicare only contributes part of the overall cost like specialist doctors, X-Rays or blood tests and the like etc etc
