Fatigue

Hey guys, let me just preface this by saying I am getting a doctors appointment, but that won't be until Monday.

I'm 26 years old, don't smoke, drink a little (a couple of beers 2 or 3 nights a week), work 9-6 5 days a week in a pretty relaxed work environment. I'm not massively stressed or anything and I generally get between 7 and 8 hours of sleep a night. I'm 5'9", and probably about 170lbs, I eat quite well, lots of vegetables and meats, maybe a bit too many carbs. I've been taking a multi-vitamin with iron supplement and Cod Liver Oil for a couple of weeks. I drink maybe 3 or 4 black instant coffee's a day (which is probably a little much) but never after 5pm and I might not drink enough water.

For the past month or so (ever since I came back from a very relaxing 1 week holiday in the Mediteranean) I've had a massive feeling of fatigue all over my body, and what I can only describe as 'brain fog' or lack of concentration. All my limbs feel incredibly heavy, I have massive issues getting out of bed in the morning and I'm just constantly mentally and physically tired. I've also had minor cold or flu symptoms (chesty cough, sore throat) and today I've started having a dull ache in my left kidney. I'm epilectic and take an anti-convulsion medication that has fatigue side effects, but I've not changed my dosage in months and never had these side-effects before.

I used to go running for 4-5km once or twice a week, now even the thought of going for a run makes me feel tired. Generally I just feel like complete crap and I'm wondering if you guys can suggest anything that might help? Like I say I am going to the doctors, hopefully on Monday but if you guys had any suggestions of something that I could be missing out on here that would be a great help.

    I'm not a doctor, of course...

    It just sounds like a disease you've picked up, really, in fact it sounds a lot like mono. It may be your spleen that's sore and not your kidney, the spleen is just a bit above the left kidney. If it's lasted a month, waiting a few more days is probably not going to make it any worse. Until you can get to the doctor though, take it easy, don't stress yourself out or anything, and don't do any vigorous exercise. If it is mono, heavy physical activity could damage your spleen.

    Sounds like you picked something up on holiday. Symptoms describe a range of diseases, probably something viral. Obviously what you've done on holiday may affect illness - Epstein Barr virus, the causative agent of infectious mononucleosis is spread by saliva for example.

    Thanks guys, I hadn't really thought of mono. Although the pain is in my lower left back, sort of just above my hip area so I'm pretty sure thats my kidneys and not the spleen. Also, call me naive but I'm 99.9% sure that neither me or my girlfriend has been sharing saliva with anyone else ;-) In fact, my girlfriend doesn't really have any of the symptoms so I guess that would rule out mono right?

    Some virus probably sounds likely, I got bit badly by a couple of mosquitos on my holiday so that could well be something to do with it.

    I also found a tick crawling up my leg hair on holiday (I was doing some hiking) but I'm pretty sure I got rid of him before he bit me (as I heard that they just fall off after biting, rather than crawling). Also when I squished him there was no blood.

    It's still possible. I never noticed the tick bite that I contracted Lyme Disease from or the distinctive rash. Be sure to bring that up with your doctor when you see him. Lyme Disease is detectable by a simple blood test and treated with antibiotics. Some of the symptoms, notably the fatigue, heavy limbs and brain fog, sound like symptoms of lyme disease.

    Once you take the antibiotics, it may take some time, but you'll return to normal.

    Your girlfriend may not have mono but that doesn't mean she's not a carrier. It's quite possible she got it as a child (when it tends to be more like a regular cold) and it's merely become contagious again, which is something that can happen. And you don't necessarily need to kiss to transmit it, a sneeze works just as well.

    But this is really here nor there. It's probably viral, go see your doctor. Everything will be fine.

    Fatigue and the other symptoms you listed are signs of stress on the body that can indicate any one of a large number of possibilities. You'd need your doctor to run some tests to find out what exactly. I wouldn't worry too much about any specifics before then, the odds of it being mono or anything else specific that we could guess here aren't so high.

    The interesting thing with Epstein-Barr virus is that it infects around 95% of the population. You only get infectious mononucleosis/glandular fever/kissing disease if infected later in life, traditionally in university students.

    Could of course be an arbovirus, with some emerging viruses in Europe, particularly in Italy. Mostly cause mild disease, and if you've got this stage you'll be fine.

    I am speaking as a microbiologist so my random guessing at some very broad, wide ranging symptoms is segued towards viruses.
