I don't really get the +1 thing for articles and such on other websites
it's visible to others who are looking at the content itself, but it doesn't show up in the feed? I don't see how it functions as a recommendation in any way in that case.

I believe the point is that it weights the search results of anyone who has you as a Google Contact (or in a Google+ Circle, presumably). A friend sent me a screenshot of a search result weighted this way last week, in fact.
