Quick and easy way to build HTML forms (For sending an email)

I have no problem with HTML, CSS, and a little PHP.
But if possible, whenever I want to put a "Contact Us" form on a web page, I'd like to be able to just generate it and point the HTML form at a location containing an email script, rather than having to do all of it by hand every time, since the forms I require are likely to be different each time.
Is there some resource I should use for this? Most services on the web I've found either store form entries on their own servers, so there's a middle-man, or they charge money.
