First, when the 'how many hours do you play' question is asked in surveys, it is usually done though a number of choices, like you did in question six. Few people log their time and would pretty much have to randomly guess anyway, so allowing them to type out a specific number doesn't give you any more accuracy. Furthermore, I'm not quite sure if asking how many hours are played in a sitting is useful data (unless of course you're working on analysing game design, in which case it is very relevent) without asking about the rate of sittings.
Question 1 should probably be a selection instead of checkboxes.
Question 6 is very odd. You're actually asking about the amount of time spent playing (per week, I assume, you forgot to put that on the question), but you're attaching the subjective concept of hardcoreness to the question for no real reason. You'd be better off either having that as a seperate question, or discarding it completely.
Question 10 asks a good deal of the respondent, requiring them to essentially mini-review every game that they've played (It's also the biggest reason why I'm probably not going to take the survey myself). It might take a bit of effort to set up, but some sort of table listing every Wii game released with selectable options from 1 to 10 might be a better idea, since it gives the same level of detail but puts less pressure on the respondent. The site you're using might not be able to handle that, though.
Finally, you've loaded the survey. It's okay for it to primarily be about the Wii, but don't start off a survey by telling the respondent that the Wii "rocks".
Also, you left out current-gen (or are they last-gen now? whatever, the terminology is stupid) consoles like the PS2, GC, etc, which people still play a lot of. I know I've probably used my Wii for Gamecube games more than for Wii games since I've gotten it. (Of course, that's also partly because I ran out of money).
I play the DS as much if not more than my Wii, and it's definitely my preferred system of the current generation. Also, it has a FANTASTIC library of games. The Wii, not so much.

Yep, no problem with the 8 I own, either. I'm a Wii whore. The Wii, Gamecube, and VC titles I've purchased since I got my Wii probably add up to more than the cost of a PS3. In Australia. Just wait till I get my 360 (when depends on when they are coming out with that elite set-up).
