Tembo tries to best to hack his way through the swarm and get to the real prize. "All these crawlies can't be good for the foundations." He hollers over the vrock's din, only to have his prediction eerily confirmed by the distant rumble. One thing at a time, Julius, one thing at a time.

Standard action: MBA vs the bug swarm. Post roll edit: That's 2 miss damage and I'll use Grim Intent to get +WEAPON damage to my next melee miss damage.
Mitch manages to get control of his screaming monkey brain just enough to come up with an escape plan. The plan essentially amounts to "stop, drop and roll", which the rational corner of his mind is pretty sure is for a different situation, but all he really cares about is that the bugs don't seem to follow.
He takes a moment to collect himself and re-connect with his inner awesome. Pure, bug-free awesome.
Well, we'll keep that queued up unless the situation changes drastically enough that you want to do something other than escape from drowning in insects with people faces.

So 11 damage to the Vrock, 5 to the Insects for each effective hit. Anything with less than 10 is weakened until end of next but I doubt anybody is there. I make one save so I'm at ongoing 5 now but still at -3 hp. Should probably look up how this deal with negative HP.
Oh, and Song of Heroes should grant +1 to everybody for one more action, I think. I maintained the song this round so it says until the beginning of my next turn. I'm down so we definitely don't trigger Final Verse effects.
Game stuff: the swarm gets hit three times, taking that damage each time; you continue rolling your saves when down, though you automatically succeed when the battle ends, justified by whatever means you can come up with; negative HP works like in 4e: you're down at <=0 hp and dead at negative half-your-max, you heal starting at 0 (same as in 4e), and you have death saves to make - 16+ you can spend a recovery, 15 or lower you fail and if you fail four times you die, and rolling a natural 20 lets you act that round. As always, people can roll healing-themed Wisdom checks to stabilize you, or feed you a potion/cast a spell/etc.
Damages all noted. You dropped three mook-units of the bugs, and did a number on the vrock as well.
Darian goes out like the encore in an Archmage-sponsored concert in Horizon, music and pyrotechnics and all. A number of beetles fall unceremoniously to the ground the exact moment that Darian does, and the vrock's face takes an even more horrific look as it momentarily reels from the kaleidoscope of pain. It might have had a stroke.

The vrock curses at Tembo and Cullen, who are holding the line and keeping it from the weaker, vulnerable types in the back. It'll just try to claw its way through.

I'm not sure. You have me annotated as hit twice, crit once, but I'm not seeing any hits against my AC at all there. I only ask because Cullen would need to rally if he did take 9 damage.
Sorry, I accidentally made some attacks against Darian (dying) and Tomas (among the shadows) so I just allocated the first of each (21, 14) to Tembo and the second (25, 23) to Cullen.

Shar grins as the bugs surround her. Thick in the action, exactly where she wants to be! Her blood sings as blue fire bursts from her hands at both the swarm around her and the vrock itself. The fire itself is weak, but damages both.

Man, I haven't rolled a single even die to hit, ever. Both do hit, but do a piddly 2 damage. Can I say that my attack against the bugs misses? I'd actually do 5 damage on a miss, versus 2 on a hit. And I'm not sure if I'm rolling damage right - the sheet says 1d6+1, but that seems really low. Should I be adding to that any bonuses or level or something?
And yeah, Geth is keeping the haterade flowing. Spells scale differently than weapon attacks, so the 1d6 part is right, but someone did mess up on the sheet though: burning hands is 1d6 + Cha so that should be 1d6+3 for you. So if you're alright with the 1 damage discrepancy we'll keep both of them hits. Actually, where's the +3 bonus to attack from? I gotta run to work but I'm not immediately recalling the source and you'd miss without it. Oh, derp, escalation die, of course.
