It happens rarely and I have no idea how to reproduce it, but occasionally when I check my bookmarks I see this:
I'm not nearly that behind. There was only one post in the thread I hadn't read. The link did take me a really old post, though.
It appears to only happen when there is actually a new post, but it happens rarely enough at all that it could easily be a coincidence.
