Citing a book, please help.

I know the format of citing a book MLA style and I have gotten this far:

King, Thomas. Truth & Bright Water.

The next parts should be Place of publication: Publisher's name, Year of Publication.

There's only one page that looks like it has any of this information but for the life of me, I can't figure out what the hell is what. Here's a picture, please help me figure this out.
