It is really simple. Put in your phone number and serial number,they send you a text with a pin, and you enter the pin into the web site.
Unfortunately you cannot use the credit to buy stuff on itunes, or for an itunes gift card.

The system is broken at this point and works on iphones you bought this week. So, even if you bought after the price drop, this may work for you.
